About this file

This is a Past Performance Questionnaire designed to evaluate contractor suitability for the Library of Congress exhibit fabrication and installation services contract (Solicitation 030ADV26R0023). The questionnaire requests assessments from current or previous customers of prospective contractors regarding their performance on similar projects. Responses must be submitted by email to Olexiy Levchuk (olevchuk@loc.gov) and Moji Adejuwon (moad@loc.gov) no later than Noon Eastern Time on April 2, 2026. The Library of Congress emphasizes that past performance is of considerable importance to this acquisition, making customer input critical to the proposal evaluation process.

The questionnaire evaluates contractor performance across ten key criteria using a five-point rating scale (Outstanding, Good, Satisfactory, Marginal, Unsatisfactory, or Not Applicable). Evaluation areas include overall contract management, quality of products and services, project phase management effectiveness, schedule timeliness and compliance, timely report submission, subcontracted work quality, key personnel competence and communication, cost performance ability, and whether major contract requirements were terminated or rescheduled. Respondents must provide their company name, point of contact information, telephone number, applicable contract number, contract type, period of performance, contract dollar value, and contract description. Narrative explanations are required for any Marginal, Unsatisfactory, or No ratings to supplement organizational knowledge of contractor performance.

Outstanding
Performance meets contractual requirements and exceeds many to the Government's benefit. The element being assessed was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
Good
Performance meets contractual requirements and exceeds some to the Government's benefit. The element being assessed was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
Satisfactory
Performance meets contractual requirements. The element being assessed contains some minor problems for which corrective actions taken by the contractor appear or were satisfactory.
Marginal
Performance does not meet some contractual requirements. The element being assessed reflects a serious problem for which the contractor has not yet identified corrective actions.
Unsatisfactory
Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The element being assessed contains a serious problem(s) for which the contractor's corrective actions appear or were ineffective.

2. Please provide narrative explanations for any Marginal, Unsatisfactory or No ratings and supplement your knowledge of the contractor’s performance with judgement of others in your organization.
