About this file

This document is a Customer Satisfaction Survey template for Federal Aviation Administration (FAA) solicitation 697DCK-7-R-00004 (also referenced as 697DCK-27-R-00004). The survey is designed to collect past performance feedback from customers of companies bidding on the FAA proposal. Completed surveys must be submitted directly to the FAA Acquisitions office via email to the Contracting Specialist, stephanie.pfeiff@faa.gov. Surveys submitted directly by the bidding company will not be accepted.

The survey collects identifying information including the name of the company submitting the proposal, the company providing the rating, the past performance contract number being referenced, type of services provided, and annual dollar amount of the referenced contract. The survey itself contains four rating questions using a five-point Likert scale (Excellent=5, Very Good=4, Good=3, Needs Improvement=2, Unacceptable=1) addressing: timely completion of services at various intervals; contractor responsiveness to needs and complaints; problem-solving capabilities and follow-up of operations management; and overall service performance satisfaction. Each question includes space for comments. The survey must be signed by the customer with a date and submitted as part of the proposal evaluation for past performance assessment.
