About this file

This is a Federal Aviation Administration (FAA) form used to collect contractor staffing and access information for potential contract awards.

The FAA 4400-48 Contractor Staffing Access Questionnaire is designed to identify the access needs of contractor employees so the FAA can determine appropriate investigation requirements and badging needs upon contract award. Contractors must complete and return the form with two primary sections of information. First, contractors provide the estimated total number of employees (full-time, part-time, and subcontractor personnel) anticipated to support the contract throughout its life, with an optional attachment listing proposed personnel. Second, contractors detail information for each labor category anticipated on the contract, using FAA-specified categories where provided or contractor-defined categories otherwise. For each labor category, contractors must identify position title(s), estimated number of employees, and answer three Yes/No questions regarding: (1) whether employees require routine access (more than one day per week) to an FAA facility for more than 180 days; (2) whether employees require access to FAA computer systems or networks; and (3) whether employees require access to FAA Sensitive Unclassified Information such as building designs, drawings, research and development materials, or operations plans. The form accommodates up to three labor categories. The questionnaire is subject to the Paperwork Reduction Act with OMB Control Number 2120-0595, with an estimated public reporting burden of approximately 25 minutes per response.
