About this file

This document is an Invitation to Bid (ITB) Bid Cover Sheet for the Empire Lake Fishing Dock & Gangway Fabrication project, issued by the Oregon Department of Administrative Services (DAS) Procurement Services on February 20, 2019, with the solicitation number S-63500-00014312. The document serves as a mandatory form for potential bidders to complete, certifying their understanding of the bid requirements, compliance with state regulations, and providing key company information for a contract opportunity related to fabricating a fishing dock and gangway.

The bid cover sheet requires bidders to affirm several critical certifications, including non-discrimination practices, tax law compliance, absence of conflicts of interest, and verification that the company is not on any federal exclusion lists. Bidders must also certify that their bid is prepared independently, without collusion, and that any false statements could result in penalties under the Oregon False Claims Act. The document is designed to ensure bidder transparency, ethical conduct, and alignment with state procurement standards, with specific attention to business practices, economic interests, and potential legal implications.
