The price proposal form requires bidders to submit detailed pricing on a per-test basis, with calculations for total annual costs and total three-year costs for both recurring and three-year cycle testing items. The form includes separate sections for renewal pricing and optional term pricing, with all pricing currently listed as zero pending bidder submission. Bidders must be certified under the National Environmental Laboratory Accreditation Program (NELAP) by the Florida Department of Health for all requested analytes and demonstrate a minimum of three years' experience in environmental water testing. The vendor must perform a minimum of 50% of work with their own forces and carry Commercial General Liability insurance of $1,000,000 per occurrence/$2,000,000 aggregate. The City reserves the right to award to the lowest, most responsive and responsible bidder or to multiple bidders at its discretion. Price increases for renewal periods cannot exceed three percent annually without documented mitigating market conditions and CPI justification submitted at least 90 days prior to term expiration.
