The price proposal form requires bidders to enter costs per test, total annual costs, and total three-year costs for all line items, with separate sections for annual recurring tests, three-year cycle tests, renewal pricing for the initial term extension, and optional term pricing. Bidders must be certified under the National Environmental Laboratory Accreditation Program (NELAP) by the Florida Department of Health for all requested analytes and demonstrate a minimum of three years' experience in environmental water testing for compliance monitoring. The vendor must perform a minimum of 50% of work with their own forces, carry Commercial General Liability insurance of $1,000,000 per occurrence/$2,000,000 aggregate, and maintain Commercial Automotive Liability of $1,000,000 combined limit. The City reserves the right to award to multiple bidders at its discretion and will select the lowest, most responsive and responsible bidder. Price increases for contract renewals cannot exceed three percent annually unless mitigating market conditions exist, and vendors must submit increase requests with CPI justification at least 90 days prior to term expiration. The vendor is responsible for all costs associated with sample kit shipment to Ocala and sample pickup and return shipment, as well as provision of sample bottle preparation and shipping containers.
