About this file

This is a Payee Data Record form (STD 204) issued by the State of California Department of Finance for the Judicial Council of California, Administrative Division, in connection with the RFP-IT-2026-212-RB Voice-To-Text Translation Program solicitation. The form serves as a required document for any non-governmental entity or individual entering into a transaction that may lead to payment from the state in lieu of IRS W-9 or W-7 forms. Vendors responding to the voice translator solution solicitation must complete and return this form to the Judicial Council's Branch Accounting & Procurement office located at 455 Golden Gate Avenue, 6th Floor, San Francisco, California 94102-3688, with completed submissions directed to roderick.bustos@jud.ca.gov or faxed to 415-865-7722. The form requires vendors to provide complete payee information including legal entity name, mailing address, email contact, entity type classification, and taxpayer identification number (TIN) such as Social Security Number, Individual Tax Identification Number, or Federal Employer Identification Number, along with California residency status declaration.

Completion of the STD 204 is mandatory for all payees seeking state compensation, and failure to provide the required payee data may result in payment delays or federal and state backup withholding reductions. The form must be signed and dated by an authorized representative of the vendor entity and returned promptly to prevent processing delays. The state uses information collected on this form to prepare Information Returns (Form 1099) in accordance with Internal Revenue Code and California Revenue and Taxation Code requirements. Nonresident vendors performing services should note that payments may be subject to California state income tax withholding unless a Franchise Tax Board waiver is provided. Government entities are exempt from this requirement, but all other vendors must complete and maintain this form on file with each state agency with which they conduct business.
