About this file

This is an Invitation for Bid (IFB) form issued by the State of Illinois for vendor submissions. The document serves as the formal bid submission template that vendors must complete and return to the state. The IFB requires bidders to submit all required documents as specified in solicitation section A.1, with the understanding that failure to meet all stated requirements will result in disqualification. The form includes sections for bidders to identify themselves, confirm compliance with solicitation requirements, and declare any exceptions to standard contract terms and conditions.

The State of Illinois Procurement Code provides various business preferences that bidders may claim, including Illinois Business Preference and Domestic Product Preference. Bidders claiming the Illinois Business Preference must complete and submit the provided affidavit, while those claiming Domestic Product Preference must designate "Domestic Product" in the alternate description field for each applicable line item. Any bidder taking exception to the Standard Terms and Conditions must submit a redlined version of those terms with their offer, with the understanding that the State may accept or reject any proposed exceptions. The Standard Terms and Conditions referenced in the solicitation are available on the Illinois Procurement Office website at https://cpo-general.illinois.gov/solicitation-and-contract-templates.html. By submitting a completed bid form, the authorized representative of the bidding company makes a formal offer to the State of Illinois that the State may accept.
