About this file

This is an Invitation for Bid (IFB) form submitted to the State of Illinois for equipment rentals. The document serves as the official bidding instrument through which vendors submit offers to perform work in compliance with the solicitation requirements. The form requires bidders to complete and submit all required documents as specified in solicitation section A.1, with failure to meet requirements resulting in automatic disqualification. The IFB includes spaces for bidders to identify their company, acknowledge understanding of submission requirements, and confirm their intent to perform in full compliance with the subject solicitation terms.

The document addresses Illinois Procurement Code preferences, allowing bidders to claim applicable preferences such as Illinois Business Preference or Domestic Product Preference, provided they submit required affidavits and documentation. Bidders may take exceptions to Standard Terms and Conditions by submitting a redlined version of the contract terms, though the State reserves the right to accept or reject any proposed exceptions. The Standard Terms and Conditions are available on the CPO General website. The form includes signature blocks and date fields for authorized representative execution, with numerous checkbox fields and text fields for bidder responses, indicating this is a template document designed for use across multiple procurement opportunities with the State of Illinois.
