About this file

This is a standard Invitation for Bid (IFB) form from the State of Illinois that appears to be a template document (version 25.1) for vendors to complete when submitting bids to the state. The document includes sections for required documentation verification, preference claims (particularly for domestic products), and acceptance of contract terms and conditions. The form references standard terms and conditions that can be accessed via the state's procurement website.

The document primarily consists of checkboxes and form fields for vendor completion, including areas for signature, date, and vendor information. The form emphasizes that bidders must meet all requirements to avoid disqualification and provides options for claiming Illinois business preferences. While this appears to be a standard form template, specific details about the actual procurement, such as scope, quantities, dates, and pricing requirements are not included in this particular document.
