Interview of: FN Velez, Andre CGC Bear Date/Time: 20MAR25/ 1330 Location: FED Interviewed by: DC1 Peña
At approximately 0100 FN Velez heard water flowing from restroom or room E121. Found water on the deck not yet at the entrance. Saw water coming from vent and placed trash can to try to catch water.
Called roommate Sn Pangallo. Sn Pangallo told FN Velez to contact OOD. FN Velez went to UPH quarter deck to get phone number. Contacted Duty watch-stander (DW) MK3 Rodriguez-Bonilla. DW Contacted OOD who then instructed DW to contact EOW, EM2 Knight. Approximately 0130 EOW and DW investigated upstairs rooms E214. EOW contacted EOWC who then instructed EOW to close the door and stand by for Base Technicians to come into work. EOW, and FN Velez remove items off floor.
FN Velez observed EOW attempting to locate isolation valves but was unsuccessful. EOW departed and FN Velez remained in the space watching TV. Finds water has entered his room. Around 0300 OOD knocked at the door. FN Velez finds water has entered his room. OOD orders Velez to leave the space because of room E122 flooding as well. Flooding was past the room and into the hallway. 0415 FN Velez leaves to CGC Bear for duty and returned at 0800 the next day to remove all personal property from E121 and into temporary room W127. No reported property damage.

Interview of: CSC Carpiso, Amin Date/Time: 20MAR25/ 1200 Location: MWR
At approximately 0100 Received a call from FN Velez stating water coming into his room at E121. CSC told FN Velez to contact Duty watch stander (DW) who then contacted EOW. 0130 CSC arrived at E121, saw the water and left once EOWC ordered to EOW to monitor. CSC then received another call approximately 0300 from SN Ingram in room E122. CSC then finds water from both rooms has spread into the hallway and approximately 1” of water on the deck starting from the Restrooms and spreading out into the berthing space. Items were removed from the floors and OOD departs scene close to 0400.
OOD Notified XO around 0530. No report of damages to personal property.

Interview of: MK3 Rodriguez-Bonilla, David Solomon Date/Time: 26MAR25/ 1430 Location: NED
On March 10th at approximately 0100 MK3 Rodriguez on duty as Duty Watch stander 2 (DW2) was contacted by OOD CSC Carpiso to investigate a report of flooding at UPH in Rm E121. Upon arrival DW2 observed pooling water on the floor of E121 Restroom not yet into the carpet of the berthing space.
DW2 could here sounds of water and identified the exhaust ventilation cover to be the site of water entering the restroom. DW2 investigated E122 (opposite wall of E121 restroom) and found no water.
DW2 contacted EOW (EM2 Knight) who upon arrival investigated the above rooms E214 and E217 for source of leak. DW2 could hear water inside the wall and felt it cold to the touch. EOW and DW2 searched for Cutt off Valves (COV) but was unable to find any. At this point water has reached the carpet of room E121. EOW orders personal property removed from the floor and electronics unplugged. EOW placed trash can to attempt to catch water from vent. DW2 Found multiple points in the wall where water was trickling in. This confirmed a busted water supply inside the wall. EOW contacted EOWC who instructed EOW to stand by and wait for civilian employees to report into work. EOW and DW2 continue to pick up personal property and converse on how to better the situation. DW2 departs close to 0300.
DW2 found no damage to personal property.

Interview of: EM2 Kight, Giana Date/Time: 21MAR25/ 1130 Location: FED
On May 10th approximately 0130 EM2 knight (EOW) received a call from MK3 Rodriguez-Bonilla (DW) reporting flooding in the restroom of UPH Rm E121. EM2 and MK3 investigated. EOW found ½” of standing water in the restroom that had already reached the carpet. EM2 stated that the water Cut off valves (COV) for sink and toilet did not stop the flow of water into the space. EOW then noticed the water streaming from the vent about the toilet. There were no sounds produced by the water. At 0145 DW and EOW knocked and entered E214 and E17 to investigate the cause of the flooding below. Residents were not present at the time. No flooding source was found. At 0200 EOW contacted EOWC about flooding in E121 stating that the source must be inside the wall above. EOWC told EOW to log it and wait for staff to arrive. EOW continued to lift items off the ground. 0230 EOW left E121 and went to UPH Quarter Deck and discussed with DW on how to better the situation. DW leaves shortly after and EOW remained in lobby for personal phone call. 0300 received a call from YN2 Fox (the oncoming DW) about flooding reported in room E122. When arrived, EM2 observed water in the hall in front of rooms E121 and E122 signifying progressive flooding. EM2 then calls EOWC (DC1 Cummings) who then states that he is enroute. EOW orders all items off the floor and securing of electrical devices that may pose a danger. OOD arrived to assist and EOWC arrives shortly after at 0345. EOWC and EOW then head to Base Central Machinery Plant (CMP) to locate isolation valve and obtain the required wrench to operate. EM2 Kight then states that the EOWC was unable to operate the valve due to condition of the valve. EOWC contacted MR. Rober Ward (public works supervisor) for further guidance. Mr. Ward informs EOWC of isolation valves inside machinery room of UPH East. EOWC locates valve and secured water supply to UPH east by observing water meter readings, and the stopping of water flow to spaces.
OOD then departs. Mr. Ward arrived on scene at 0400 with Carpet Vacuum to recover water. EOWC orders residents relocated, property removed from floors and electrical devices that pose a risk unplugged.
No Damage to personal property reported or found. EOW departs scene at 0430.

Interview of: DC1 Cummings, Donald Date/Time: 26MAR25/ 1115 Location: FED
On March 10th at approximately 0130 DC1 Cummings (EOWC) received a text message from EM2 Knight (EOW) stating: “I’m pretty sure there’s a busted pipe in the wall its coming from the dry wall and ceiling it’s flooding pretty bad and it’s flooding the room from the walls”. EOWC calls EOW and was told about the water amount and that a bucket was placed to catch water trickling into the restroom of E121. EOWC then told EOW to monitor and notify if the situation gets worse. At approximately 0350 EOWC received a text message from EOW stating “The flooding has gone into the next room over as well E122), followed by “I think I found it” EOWC calls EOW to notify that he’s enroute.
Approximately 0430 EOWC arrives on scene and observes the extent of the water flooding into the hallway from the restrooms of E121 and E122. EOW and EOWC head the Central Machinery Plant (CMP) to identify the location of the water supply cut off valve (COV) and retrieve a wrench to do so.
EOWC attempted to close water supply COV but was unable due to mud and water obstructing the COV operation. EOWC contacts Public Works Supervisor (Mr. Robert Ward) who informs EOWC of alternate COV located inside UPH East Machinery Room. EOWC located said valve and isolates water supply to UPH east by confirming that water trickle has stopped, and the water supply meter read zero. Mr. Ward supplies a Carpet wet vacuum and EOWC began to recover flood water as EOW and DW continues to remove items from floor. At around 0600 Base Portsmouth civilian work force arrived and took over cleaning efforts and repairs. No damage to personal property was found during this time.

Interview of: YN2 Fox, Jennifer Date/Time: 20MAR25/ 1100 Location: FED
On March 10th YN2 Fox (DW1) received a call at approximately 0330 from SN Ingram in UPH Rm E122 stating that his room was flooding badly. DW1 then notified EOW (Em2 Knight) who relayed that she was already next door in E121. DW1 arrived on scene at approximately 0340. DW1 could hear and see water actively entering the berthing of E121 and E122 from each room’s restroom. EOWC arrived on scene around 0400 and informed of water supply line failure within the walls above in E121 and E122.
EOWC makes decision to secure water supply to UPH East. DW1 departed and reported to gate duty at 0600-0715. DW1 returns to flood to assist. Remediation effort were underway. At 0800 DW1 contacted the command of each resident to inform them of the flooded rooms as well as base XO. Command master chief and Base CO visited site to view extent of the damage. New rooms and key cards were issued to residents of E122 and E121 later that day. On Wednesday March 12th the effected residents were ordered to remove all personal property from the flooded rooms and into their temporary rooms. FN Veles, SN Colon, SN Pangallo were located to W127. SN Miller, SN Mcclelland, SN Ingram we relocated to W126. YN2 Fox who also is assistant UPH Manager, orders removal of furniture items and confirmed that Servepro was onsite and conducted treatment on Thursday March 13th. Odors were eliminated. Once new carpet is installed, each effected resident will return to their original rooms.

Interview of: SN Ingram, Dacoda Date/Time: 21MAR25/ 1100 Location: FED
On March 10th at approximately 0300 SN Ingram woke from E122 at the sound of water coming from restroom. SN Ingram went to UPH front desk to get the contact number for the Duty watch stander. SN Ingram did not see water in the hallway and the door to E121 was shut at that time. The Duty watch stander, YN2 Fox (DW1) stated she was enroute and would notify EOW. SN Ingram text messaged Roommates about the situation. EOW and DW1 arrived on scene and orders items from floor and unplugged electronics that pose a risk. EWO knocks on room E121 to send SN Velez to sleep on cutter (CGC Vigorous) SN Ingram departed scene at 0400 to report in for duty on board CGC Bear. The following day Sn Ingram was relieved from duty and removed all personal property from Room E121 and into W126. No report of damage to personal property.

Interview of: MR. Rober Ward Date/Time: 26MAR25/ 1030 Location: FED
On March 10th at approximately 0420 Mr. Ward (Public Works Supervisor) received a call from DC1 Cummings (EOWC) requesting water Cutt of Valve (COV) to UPH East. Mr. Ward informed EOWC about COV located inside machinery space of UPH East. 0439 EOWC notified Mr. Ward that water was secured to UPH east. Mr. Ward arrive on scene at 0530 and met with EOWC extracting water with a shop vac. EOWC then briefed Mr. Ward who then confirmed that Civilian techs were enroute. 0600 techs on site and water was resupplied to the building to pinpoint the location of the pipe leak. A mirror was removed from the restroom wall of E214 and Mr.Ward identified the source of the water was coming from a failed water hammer arrestor – A device commonly installed on water supply lines to eliminated knocking pipes during water supply demands. Water was then secured. Mr. Ward attempted to remove the water hammer arrestor but was unable to due to a soldered connection. Mr. Ward then directed his team to crop out a section of the brick wall to allow safe access for hot work on the water hammer arrestor.
Around 0800 the failed component was removed, and cracks were found just above the device’s connection point. A new device was soldered in place of the old and water was restored at approximately 0900. A metal hatch was installed to cover the hole that was made to access the water hammer arrestor.
Box fans were put in place to move air and dry the carpet. Residents were relocated and all furniture was removed for Servepro who serviced the effected spaces on Thursday 13 March 2025. Mr. Ward has plans scheduled his team to replace the carpet of the effected spaces on 14April25
