If its going to be an IFB then those usually come to be via opengov and we fill out all of the insurance requirements through there.
What table is Jesse referring to in his initial email? Typically anything that engineering sends to us with a request for insurance requirements, they also supply the document that they are using in the agreement that will apply to the project.

Good day, We are preparing to proceed with the bidding phase for the above-captioned project. Please provide us with the insurance requirements, based on the project scope detailed below.
This scope of this project includes the dredging of Bartlett Lake, located along 4th Street and 22nd Avenue South. Approximately 22,806 cubic yards of material will be removed from the lake across an area of 2.62 acres. This will provide additional capacity for managing stormwater in the basin. The material will be staged and dewatered in the adjacent retention area with the contractor taking the appropriate environmental precautions. The contractor is responsible for removal of the material from site and disposal, whether that is through reuse or landfilling it is up to the contractor.
Estimated Construction Value:
$2,721,434 - $5,502,842 * the variation in cost is due to the potential for reuse of the material which would cost the $0 for the City.
