About this file
This is an Insurance Requirements checklist for the City of New Smyrna Beach, Florida, establishing mandatory insurance coverage specifications for a consultant providing insurance consulting services. The City seeks a qualified insurance consultant firm to review the municipality's allocation of insurance assets, premiums, and coverage needs; assist with developing and administering a Request for Proposals for liability and related insurance coverages; evaluate proposals received; provide recommendations on prior allocations; and develop annual insurance premium allocations. The contract term and renewal options are not specified in this requirements document. Respondents must submit qualifications within the timeframe established by the City, with incomplete or unresponsive submissions subject to rejection. Award of the contract is contingent upon receipt of required insurance documents within fifteen calendar days of City notification, with an additional five-day grace period permitted if deficiencies are identified. If corrected documentation is not received within twenty calendar days, the contractor will be in default and the award will be rescinded unless the City extends the deadline.
The consultant must maintain comprehensive insurance coverage throughout the contract period, including Commercial General Liability ($1,000,000 per occurrence), Automobile Liability ($1,000,000 per person/$1,000,000 per occurrence), Workers' Compensation as required by Florida Statute 440, Professional Liability including Errors and Omissions ($2,000,000 per occurrence with three-year tail coverage post-completion), Pollution Liability ($1,000,000 per claim), Cyber Liability ($1,000,000 per claim), and other specialized coverages as applicable. All insurance carriers must be rated no less than "A-" by A.M. Best Company and authorized to conduct business in Florida. The City of New Smyrna Beach must be named as an additional insured and loss payee on all policies, with certificates of insurance submitted to the City's mailing address at 210 Sams Avenue, New Smyrna Beach, Florida 32168. Insurance policies must include thirty-day cancellation notice provisions and require thirty days' written advance notice to the City for any coverage modifications. The consultant is responsible for maintaining active insurance coverage for the entire contract duration and any option years, with renewal certificates submitted at least thirty days before expiration; failure to provide timely renewal certificates may result in contract suspension and potential termination.
