About this file
This document is a Requirements for PIV Badge Sponsorship instruction set from VA Salt Lake City, dated 03/26/2025, detailing the precise steps a contractor must follow to obtain a Personal Identity Verification (PIV) badge. The document outlines four mandatory documents that must be electronically completed: a TMS 2.0 Training completion certificate, a Contractor Background Investigation Request, a Declaration for Federal Employment, and a Self Certification of Continuous Service. Specific requirements include completing forms electronically, printing and wet-signing certain documents, scanning them back to electronic format, and submitting all materials via an encrypted S/MIME email to the Contracting Officer's Representative.
Critical procedural details include presenting two specific forms of identification at a fingerprint appointment (driver's license plus either a Social Security Card, birth certificate, or passport), completing eApp system requirements within seven days of receiving login credentials, and carefully following all instructions to avoid document rejection. After document processing, the contractor will receive two separate email messages with a user ID and temporary password, and will eventually receive an automated email notification about PIV badge sponsorship with instructions for scheduling a fingerprint appointment.
