About this file
This document is a Semi-Annual Elevator Inspection Report dated September 23, 2025, from GreenEfficient, Inc. to Kenneth Ballard at the Department of Veterans Affairs for the Garland VA Medical Center located at 2300 Marie Curie, Garland, TX 75042.
The report documents inspection findings for 14 elevator units (P1 through P14) at the facility. Two units—P2 and P14—were out of service at the time of inspection and are excluded from the report; annual inspections will be performed once these units are returned to service. The remaining 12 operational units contain multiple code violations referenced to ASME A17.1 and A17.2 standards. Common violations across nearly all units include: failure to provide fire logs and periodic test tags for motor control panels (MCP's) and failure to repair emergency phones. Additional specific violations identified include: defective fire phase indicators and buzzers (P3, P5, P6), broken hoistway door spirators requiring self-closing door repairs (P4), defective emergency lighting and alarms (P8, P11), restricted car door openings requiring door restrictor repairs (P12, P13), and water or oil accumulation in the pit area (P13). The report requests that the Department of Veterans Affairs advise when units P2 and P14 return to service to schedule their annual inspections.
