About this file
This is an evaluation rubric and score sheet for the Wisconsin Department of Workforce Development's Project SEARCH Request for Applications, designed to assess Community Rehabilitation Agency applicants seeking to operate at five existing Project SEARCH sites across Wisconsin: Andis Company in Racine, Children's Hospital of Wisconsin in Wauwatosa, Waukesha Prohealth in Waukesha, and Froedtert locations in West Bend and Wauwatosa. The evaluation process includes mandatory pass/fail criteria requiring applicants to provide clear contact information for a single point of contact and demonstrate a statewide service agreement with the Department of Vocational Rehabilitation that includes job placement and development, job and task analysis, systematic instruction, and supported employment services as outlined in the approved 2020-2022 Service Agreement.
Scoring criteria allocate up to 100 points across five weighted categories: Educational Partnership (10 points), assessing the strength of confirmed versus potential partnerships with school districts, career technical schools, or community colleges; Capacity and Commitment During COVID-19 (30 points), evaluating collaborative partnership history and service continuity planning; Qualifications of Personnel (20 points), based on management and key personnel experience in transition or Project SEARCH programs; and Letters of Support (20 points), which must include mandatory support from the DVR Director for the applicable Workforce Development Area and all long-term support partners in the area (MCO and IRIS), with additional optional letters from business partners and educational partners. Higher point values are awarded for demonstrated current or past success in Project SEARCH partnerships and confirmed relationships over potential ones.
