About this file

This is an Applicant Warranties and Representations attachment for the Project SEARCH Request for Application issued by the Wisconsin Department of Workforce Development (DWD). The document establishes the mandatory certifications and representations that applicants must provide when submitting proposals to serve as Community Vocational Rehabilitation Partners or Education Partners at five existing Project SEARCH sites located in Racine, Wauwatosa, Waukesha, and West Bend. Applicants must affirm their legal incorporation and authorization to conduct business in Wisconsin, their qualification to operate in relevant jurisdictions, and their compliance with all applicable laws, regulations, and tax obligations. The application process requires submission by an authorized organizational representative whose signature, title, contact information, and date must accompany the warranty statements.

The warranty document establishes that applicant grant funds must supplement rather than supplant existing planned or allocated funding, and applicants acknowledge that all records produced or collected under the Request for Application are subject to disclosure under Wisconsin Public Records Law. No specific pricing terms, contract term length, or renewal options are detailed in this attachment, which serves primarily as a certification mechanism to ensure organizational eligibility and accountability. The document is administered by the DWD Division of Operations in Madison and requires submission to the designated departmental contact for consideration in the competitive selection process.
