About this file

This is a Questions and Answers document for RFQ IHS1528577 regarding fire-door replacement and repair work at the Hopi Health Care Center located on the Hopi Reservation in Arizona.

The project involves repair of twenty (20) fire doors and replacement of two (2) fire doors. Since no itemized parts schedule was provided in the solicitation, the Government accepts quotes priced on complete repair kits for all twenty doors, with each door priced to include the full complement of potential repair components (gap correction kit, smoke seals, strikes, shims, coordinator, and hardware replacement as applicable). The two replacement doors measure 83 5/16" H x 35 5/8" W x 1 ¾" T and must match veneer to provided images; interested parties should contact Kerri.Gilmore@ihs.gov for images. The 30-calendar day period of performance does not account for material procurement lead time; however, contractors may begin repairs on the twenty doors while replacement door slabs are on order. A site visit is scheduled for August 28, 2026 at 11:00 AM Arizona Time at the front lobby, limited to two representatives per company. The work is subject to Construction Wage Rate Requirements (CWRR) WD # AZ20260027, with the Carpenter classification at $33.55 per hour base wages plus $14.17 per hour in fringe benefits. An amendment incorporating the wage determination and required labor clauses will be issued. Contractors must comply with Hopi Tribal Employment Rights Ordinance (TERO) requirements, including obtaining a business license from the Hopi Tribal Employment Rights Office and paying associated TERO fees prior to starting work. The prime contractor is responsible for TERO compliance and all applicable tribal fees and taxes must be included in the firm-fixed price on the SF1442.

RFQ IHS1528577 – Questions and Answers

1. Repair hardware schedule. Section 3.2 of the SOW states the Contractor shall provide all replacement hardware "listed in the attached schedule of required repair components," and Attachment B of the SOW references "the attached parts list." No itemized parts schedule was included in the solicitation package. Can the Government provide the inspection report or itemized hardware schedule identifying the specific components and quantities required for each of the twenty

(20) repair doors? See #2

2. Pricing basis if no parts schedule is available. If the itemized parts schedule cannot be provided, would the Government accept quotes priced on the basis of complete repair kits for all twenty (20) doors i.e., each door priced to include the full complement of potential repair components (gap correction kit, smoke seals, strikes, shims, coordinator, and hardware replacement as applicable)? YES We ask so that all offerors price to a common assumption and the Government receives comparable quotes covering the full scope.

3. Replacement door details. For the two (2) replacement doors: (a) are these single-leaf doors or door pairs? (The specified automatic flush bolt is typically pair hardware.) (b) Can the Government provide door dimensions, handing, veneer/finish requirements, and whether the slabs must be factory machined for existing hardware locations? 83 5/16H, 35 5/8 W 1 ¾ T match veneer to images, I do not know about factory machining. For images, contact Kerri.Gilmore@ihs.gov

4. Period of performance vs. material lead time. Fire-rated wood door slabs are special order items with typical lead times of 2–4 weeks. Does the 30 calendar day period of performance account for material procurement lead time? NO Alternatively, would the Government permit the Contractor to begin the twenty (20) door repairs while the replacement slabs are on order? YES

5. Site visit. Will the Government offer a site visit prior to the quotation due date?

Scheduled for August 28, 2026 at 11am (Arizona Time) Meet in the front lobby at 11:00 on Friday Aug 28th Park out front of the main entrance. Site visit limited to maximum of two (2) represenatives from each company.

6. Tribal fees and permits. Can the Government confirm which tribal fees, permits, and worker registration requirements (e.g., Hopi TERO) apply to performance at the Hopi Health Care Center, so these may be accurately included in our firm-fixed price?
