About this file

This is a self-certification form for Indian Owned Economic Enterprise representation issued by the United States Department of the Interior in July 2013.

The form requires offerers to self-certify whether they meet the definition of "Indian Owned Economic Enterprise" as defined in 1480.201 under the authority of 25 USC 47, the Buy-Indian Act. Contractors must maintain eligibility status throughout three critical periods: at the time of offer submission, at the time of contract award, and during the full term of contract performance. Any loss of eligibility during these periods requires immediate written notification to the Contracting Officer. The form includes a checkbox provision allowing offerers to indicate whether they do or do not meet the Indian economic enterprise definition. Signatory fields require the business name, certifying signature, SAM UEI number, and printed name. The form warns that false or misleading information submitted in consideration of an award set-aside under the Buy-Indian Act violates federal law and is punishable under 18 U.S.C. 1001, with false claims during contract performance subject to penalties under 31 U.S.C. 3729-3731 and 18 U.S.C. 287.
