About this file
This document is a sample license decal from the Michigan Department of Natural Resources' Off-Road Vehicle (ORV) Program. The decal serves as proof of payment for the required ORV license, which is mandated under Act 451, PA. 1994 as amended. The license is valid for one year, expiring on March 31, 2019, and carries a fee of $10.00. The decal includes placement instructions requiring users to remove the license from its paper backing and affix it to a clean, dry surface on the rear of the vehicle, preferably centered on a fender or bumper. The license is non-transferable and becomes void if removed from the vehicle.
Violations of the ORV license requirements may result in fines and/or imprisonment as stipulated by state law. The decal features a hologram element for security purposes and is identified by form number IC9405-2 (Rev. 12/15/2015). This appears to be a sample document provided to demonstrate the appearance and placement requirements of the official ORV license decal to vehicle owners and operators in Michigan.
