Incident & Investigation Information Management System (I3MS) Modernization Incident Reporting & Review User Stories
Incident Reporting Requirements (External View)
As an authorized user I want to access a secure, user-friendly, web-based application so that I can submit, search for, view, edit, and/or review the information for an incident or oil spill follow-up information report that must be reported to BSEE.
Acceptance Criteria:
· The application will be accessible both as an external/public-facing view, which allows Industry or BSEE acting as an agent of an Operator to submit, update, and view information about an incident or oil spill follow-up information report, and as an internal view, which permits BSEE reviewers to view, review, and accept the information the Lessee or Operator representative submitted, as well as to communicate with and request more information from them. The internal view will also permit BSEE investigators to view and enter additional investigation data for the incident or oil spill follow-up information report.
· Authorized users include Lessee or Operator representatives, who can be personnel of an Oil & Gas energy company or Renewable Energy company, and internal BSEE and BOEM users. Separate security roles/groups with specific entitlements will be needed for multiple Operator user types, multiple BSEE user types, and BOEM user types.
· Each Lessee or Operator representative must be granted entitlements to submit, edit, and/or view incident and oil spill follow-up information reports on behalf of a Lessee, Operator or Operators.
· Note: Lessee or Operator representatives should register and be granted access to our systems (with Administrator or General User roles), consistent with the processes described in the BSEE eWell and TIMS Web Application Manual: https://www.bsee.gov/sites/bsee.gov/files/bsee-policy-manual-section/ocs-operations/ewell-permitting-and-reporting-system-application-manual.pdf.
· Lessee or Operator representatives should only be able to view and access the regions, report types, and companies which they are authorized/entitled/permitted to view.
· Separate security roles/groups with specific entitlements will be needed for multiple BSEE and BOEM user types for each region, report type, incident report review, oil spill follow-up information report review, and investigations.
· BSEE and BOEM users can have access to incident and investigation information for all regions and all report types combined if granted the respective permissions/entitlements.
· Views/dashboards will be customized based on the roles or entitlements assigned.
· Single sign on is requested for internal users.
As a Lessee or Operator representative or Internal User (Agent For) I want to select an Operator which is associated with an incident or oil spill follow-up so that I can report a new incident or oil spill follow-up or view/update an existing report associated with the Operator.
Acceptance Criteria:
· Display the following value list options from which the Lessee or Operator representative or Internal User (Agent For) can select an appropriate Operator.
· For each Operator, display its Company Name and Number as a concatenated value.
· Note: For the Lessee or Operator representative, they can select from an Operator value list option that includes only qualified companies with which they are associated.
· Note: For the Internal User (Agent For), they can choose from an Operator value list option that contains all qualified companies, with an option to also view and select from a value list option of non-qualified companies if needed.
As a Lessee or Operator representative or Internal User (Agent For) I want to create a new incident or oil spill follow-up information report for a specified Operator so that I can report the incident or oil spill follow-up and choose the appropriate report type and region for the report.
Acceptance Criteria:
· Display an option to create a new report.
· Display a value list option from which the Lessee or Operator representative or Internal User (Agent For) can select an appropriate Region, with Alaska, Atlantic, Gulf of Mexico (GOM), and Pacific as values.
· Display a value list option from which the Lessee or Operator representative or Internal User (Agent For) can select an appropriate Report Type, with Oil & Gas Incident Report, Renewable Energy Incident Report, and Oil Spill Follow-up Information Report as values.
· Note: For incident reporting, the Report Types include Oil & Gas Incident Report and Renewable Energy Incident Report. For follow-up reporting, the Report Type includes Oil Spill Follow-up Information Report.
· Note: The appropriate data fields will be displayed for the selected Report Type in the report, e.g., only data fields relevant or related to an oil and gas incident will be shown in an Oil & Gas Incident Report.
· When a new report is created, display its Status as “Draft.” This Status and the associated Status Date and Time and Username (of the user who created the report) will be populated automatically for the report.
· Note: After receiving an immediate report of an incident via telephone or email, an Internal User (Agent For) can create an incident report on behalf of an Operator so the BSEE Reviewer and/or Investigator can enter information in the internal view of the investigation part of the incident report if the Lessee or Operator representative has not yet created the report. In those situations, the system will have an optional function for BSEE to send an email message to inform the Lessee or Operator representative of the created report and for them to complete the remainder of the report. The Internal User (Agent For) would have the option to choose from one or more authorized representatives of the selected Lessee or Operator to receive this notification.
· Note: If an Internal User (Agent For) creates an incident report, the BSEE Reviewer and/or Investigator can complete the investigation part of the incident report even when the incident report is in Draft status. However, the Lessee or Operator representative cannot cancel an incident report that was created by an Internal User (Agent For). Rather than cancel, their only option will be to request a withdrawal.
· Provide an option that saves the report automatically while in Draft status so the Lessee or Operator representative or an Internal User (Agent For) can exit the application and return to complete the report prior to submitting it.
As a Lessee or Operator representative or Internal User (Agent For) I want to find an incident or an oil spill follow-up information report associated with a specified Operator and for which I have authorized access so that I can view, edit, submit, resubmit, and/or request a withdrawal of a report that has not been accepted, or view reports of previously accepted reports.
Acceptance Criteria:
· Display reports which the Lessee or Operator representative or Internal User (Agent For) has permissions or entitlements to view or edit.
· Note: Other authorized representatives of the Operator, with appropriate permissions or entitlements, who did not create the incident or oil spill follow-up information report can view the report and edit it on behalf of the Operator.
· Reports listed should be easily distinguishable among Oil & Gas Incident Reports, Renewable Energy Incident Reports, and Oil Spill Follow-Up Information Reports.
· The Lessee or Operator representative display should only show those report types which they are authorized to view and access.
· By default, for incident reports, display those that need the attention of the Lessee or Operator representative or Internal User (Agent For) first, including ones with the following statuses: Draft, Unsubmitted, Returned, and In Rework.
· By default, for oil spill follow-up information reports, display those in Draft status first.
· Also display the following options from which the Lessee or Operator representative or Internal User (Agent For) can search, filter, and sort incident and oil spill follow-up information reports. Note that the specified meta data fields will be displayed for each appropriate Report Type.
· For an Oil & Gas Incident Report, include Company, Incident Date (MM/DD/YYYY), Incident Time, Surface Area, Surface Block, Surface Lease, District, Region, Facility Type (Platform, Rig, Pipeline, Platform/Rig, Platform/Pipeline), Platform Name, Rig Name, API Number, Incident Type(s), Status, Status Date, and Follow-Up Due Date, and any additional fields provided by the Product Owner and/or SMEs.
· For an Oil Spill Follow-up Information Report, include the same meta data fields used for an Oil & Gas Incident Report.
· For a Renewable Energy Incident Report, include Company (Lessee or Operator), Contractor, Incident Date (MM/DD/YYYY), Incident Time, Surface Area, Surface Block, Surface Lease, Project Name, Region, Facility Type (Facility, Vessel, Cable, Facility/Vessel, Facility/Cable, Vessel/Cable), Facility Name, Vessel Name, Vessel’s IMO/Registration Number, General Activity(ies), Work Process, Incident Area, Incident Type(s), Status, Status Date, and Follow-Up Due Date, and any additional fields provided by the Product Owner and/or SMEs.
· For the Follow-Up Due Date field, it needs to be color-coded based on the length of time since the Incident Date. For example: Black if today’s date is 0-10 days after the Incident Date, Orange if today’s date is 11-15 days after the Incident Date, and Red if today’s date is 15+ days after the Incident Date.
· From the default and/or filtered search results, the Lessee or Operator representative or BSEE Agent can select a report and they will be brought to the respective incident report to view, edit, submit, resubmit, and/or request a withdrawal.
· From this dashboard, the Lessee or Operator representative can also select to view a PDF of the incident report in its current status.
· Note: The content of the data to include in the PDF report will be provided by the Product Owner and/or SMEs.
