About this file
This document is a Contractor's Application for Payment form for the Highway 103 Waterline project, involving the City of Idaho Springs in Colorado and engineered by JVA, Inc. The project involves the construction of approximately 16,500 feet of 16-inch C900 PVC waterline, with the payment application form serving as a standardized document to track work completed, materials stored, and payment progress. The form is an EJCDC (Engineer's Joint Contract Documents Committee) standard template designed to provide a comprehensive overview of contract work status, including bid items, quantities, unit prices, and percentage of work completed.
The payment application form contains blank fields, indicating this is a template or preliminary document not yet filled with specific financial details. The form allows for tracking original contract price, change orders, total work completed, retainage, and amounts eligible for payment. It includes sections for contractor certification, engineer recommendation, owner approval, and potential funding agency review. While no specific dollar amounts are populated in this version, the document provides a structured framework for documenting and requesting payment throughout the project's lifecycle, with provisions for tracking both unit price work and potential stored materials.
