Proposal Submission Instructions:
Administrative and Other Support Services Contract Solicitation Number: HT9404-13-R-0003 Description: Administrative and other services to support USU in its daily operations in positions including but not limited to administrative, professional, clerical, technical, scientific, medical and laborer positions.
The resultant award will be a firm fixed priced labor hour five (5) year Indefinite Delivery Indefinite Quantity (IDIQ) contract as a total small-business set-aside under NAICS code 561320, Temporary Help Services with a size standard of $25.5M. Vendors must be registered in the SAM.gov database (www.sam.gov) as a small business under NAICS code 561320 by the due date for proposals.
Proposals submitted in response to this request for proposals (RFP) shall be hardcopy and mailed to or delivered to Carolyn Randolph, Contract Specialist at the following address:

The proposal must be submitted to the address listed above. Email proposals or proposals submitted via fax will NOT be accepted.
Proposals are due by 1:00 P.M. (EST) on June 7, 2013. The late proposal instruction found in FAR Part 15.208 does apply to this solicitation. LATE PROPOSALS WILL NOT BE ACCEPTED. It is the offeror’s responsibility to ensure proposals are received by the Government office designated in the solicitation. Once your proposal is received you will receive a confirmation email.
Proposal Organization:
Font Size: 11 Minimum Font Type: Calibri or Times New Roman Margins: “normal”, 1 inch all around
Proposals should be submitted in the following format and must use the numbering format as outlined:
· Coversheet – Must be on top of the submission package so confirmation email can be sent
· Company Name
· Solicitation Number
· Acknowledgement of Latest Amendment
· POC Information – Name, Title, Email and Telephone
· Company DUNS
· Statement that Proposal is Valid for 90 Days
· Intro Letter (optional) – 1 page Maximum – Include mention of any teaming agreements and who they are.
Technical Proposal:
I. Corporate Experience - No page limitations – Corporate Experience should be presented in the following order using the numbering format as provided in the evaluation factor as follows:

| I.d) | Discussion of making up missed hours |
| I.e) Occupational Health and Safety (OHS) plan | |

| I.g) Accommodation for J-1 (Exchange) and F-1 (Student) Visas | |
| I.h) Proposed Benefits packages spreadsheet | |
| I.i) Proposed Benefits migration plans | |
| I.j) Discussion of teaming arrangement |
II. Key Personnel – See page limitations below – Key Personnel should be presented in the following order using the numbering format as provided in the evaluation factor as follows:

| II.b) Other supporting Key Resumes – 6 page limitation |
III. Past Performance – See page limitations below – Past Performance References should be presented in the following order using the numbering format as provided in the evaluation factor as follows:

| III.b) Past Performance References SUB (If Applicable) – 2 page limitation |
| III.c) Past Performance Evaluations PRIME (optional) – 5 page limitation |
Price Proposal:
· Price Proposal – 1 page Maximum – Offerors must propose price in the form of one Multiplier for each of six (6) subCLINs, using the format and subCLIN order as outlined in the RFP Offerors must submit the required number of copies of the Technical and Price proposals:
· Number of copies of Technical Proposal = seven (7)
· Number of copies of Price Proposal = two (2)
Offeror must submit one (1) copy each of completed copies of two fill-in provisions included in the PROVISIONS and CLAUSES document attachment
· FILL-IN Provision: 52.209-7 Information Regarding Responsibility Matters
· FILL-IN Provision: 252.209-7995 Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction under any Federal Law—Fiscal Year 2013 Appropriations.
