MQS Acquisition RFP HT0050‐16‐R‐0001 Consolidated Questions and Answers
Number Topic Question/Comment RFP Reference Answer
3001 Deliverable Would the DHA please provide a word version of Attachment 3 to allow offerors to fill out the form correctly. For example, section 3 of the form, Location column, will not allow the listing of more than 1 location while in PDF format, as the cell will not expand.
Attachment 3 MSWord documents of Attachments 3, 4 and 6 have been posted under solicitation HT0050‐16‐R‐ 0001 found at FBO.gov. These documents may be used in lieu of the original PDF files. No other changes to the attachments have been made.
3002 Deliverable Would the DHA please provide a word version of Attachment 4 to allow offerors to fill out the form correctly. For example, the address column, will not allow the listing of the entire address while in PDF format, as the cell will not expand.
Attachment 4 See response to question 3001.
3003 JC Certification The solicitation states that any company and/or Prime contractor in a JV submitting a proposal must have organizational certification by The Joint Commission (TJC) for Health Care Staffing Services. We’ve reviewed the requirements for TJC certification for Health Care Staffing Services and it’s highly geared for companies that provide temporary/short‐term staffing (i.e. Locum Tenens, Per Diem, and travel staffing) services. Please provide clarification.
Section 52.212‐1 Instructions to Offerors‐‐ Commercial Items, 7. VOLUME II – TECHNICAL Page 129:
The Joint Commission certification for Health Care Staffing Services is not mandatory unless the Prime contractor is claiming those points on the self scoring technical capability worksheet (found at Attachment 5). For a Joint Venture, only one of the companies in the Joint Venture proposing as a Prime Contractor needs to have the organizational
3004 Deliverable Attachment 3 was provided in PDF such that modification cannot bemade to the line spacing or headers size. Could the attachments be provided with fillable indexes so that we can fill out the PDF blanks?
Attachment 3 See response to question 3001.
3005 Deliverable As we are unable to manipulate the Attachment 3 schedules when the List of Labor categories exceed 9 lines, should we submit two forms? Can the government provide forms that accommodate additional lines being added?
Attachment 3 See response to question 3001.
Numb er Topic Question/Comment RFP Reference Answer
3006 Unit Security Performance Work Statement, 5.2.9.5.3.1. "...The Contractor shall ensure PWS 5.2.9.5.3.1., 5.2.9.5.3.3., and This will not be a reimbursable expense. Security and travel requirements Manager the following is accomplished prior to the required or planned start date 5.2.9.5.3.5. will be outlined inMILDEP TOPRs and subsequent TOs.
in the MTF:" Paragraph 5.2.9.5.3.3. States "Schedule an appointment with the Unit Security Manager at the MTF where services are provided."
5.2.9.5.3.5. States "The Appointment with the Unit SecurityManager is necessary to ensure:" These actions have to be taken prior to hiring and could impose a significant travel cost on a Contractor whomay be required to submit multiple personnel to specific Air ForceMTF's. Will this be a reimbursable expense?
3007 Sample TOPR Page 9 ‐ 52.212‐4 (y) Task Order Proposal Request (TOPR) Process ‐ States:
"TOPRs will include, at a minimum, the following:" The list contains item g) FAR 52.222‐46, Evaluation of Compensation for Professional Employees Service. Inclusion of this clause in a solicitation requires offerors to submit a total compensation plan setting forth salaries and fringe benefits proposed for the professional employees who will work under the contract. Does solicitation Volume IV therefore require offerors to submit a Compensation Plan in support of Direct Labor and Total Task Order Price?
Page 9; 52.212‐4 (y) Task Order Proposal Request (TOPR) Process
No. 52.212‐4 (y) is the clause that outlines the "Task Order Proposal Request (TOPR) Process”. As applicable, it would only pertain to Task Orders solicited by the ordering activities AFTER the MQS contracts are awarded. Therefore, FAR Clause 52.222‐46 does not apply to Volume IV submittals. As stated in the RFP, there will be no Task Orders awarded as a result of the sample market segment TOPRs pricing evaluation.
3008 Deliverable The Past Performance Questionnaire Attachment 6 is not a fillable form.
Can the government provide this form in a fillable format?
Attachment 6 See response to question 3001.
er Topic Question/Comment RFP Reference Answer
3009 Joint Venture 52.212‐1 Instruction to Offeror, section 2 (General Instructions) page 124 states: If the Offeror is part of a Joint Venture, the Offeror shall provide a copy of the Small Business Administration (SBA) approved Joint Venture Certification documenta t i on .
Would the Government confirm that, SBA approved Joint Venture Certification documentation is only required when the offeror is being considered for contract award and not part as part of offeror’s initial submission of proposal? SBA is asserting that IAW 13 CFR 124.513, means that a Joint Venture must be approved by the SBA prior to award of the contract, not at the time of submission of proposal.
Page 124; 52.212‐1 Instruction to Offeror, section 2 (General Instructions)
See solicitation Amendment 0001. The Government will not require submission of certification documentation.
3010 Joint Venture Does the Government distinguish between SBAMentor‐Protégé Joint Ventures and Joint Ventures for this effort?
Page 130 TheGovernment does not distinguish between the SBAmentor‐protégée JV and a joint venture for this effort.
3011 Joint Venture Does the government distinguish between a prime contractor – subcontractor relationship and members in a SBA approved Mentor Protégé Joint Venture?
Page 130 TheGovernmentwill only consider the business relationships and partnerships identified on the Contractor Team Organization (Attachment 4).
3012 Joint Venture Will the Government confirm that the SBA approvedMentor‐Protégé Joint Venture is in fact the Prime offeror and not one of the JV members?
Page 130 For the purposes of this acquisition, the SBA approvedMentor‐Protégé Joint Venture will be considered one entity, the Prime contractor, as registered in SAM.gov.
3013 Past Performance Reference
Does the Government consider all past performance bymembers in a SBA approved Mentor‐Protégé Joint Venture to be prime Past Performance?
Page 130; paragraph 8. Volume III Yes. All information frommembers whomake up the Joint Venture will be considered for the purposes of this evaluation.
er Topic Question/Comment RFP Reference Answer
3014 Joint Venture Page 130 States: “For Organizational certification section, the Offeror, must provide a copy of The Joint Commission (TJC) certification in Health Care Staffing Services for the prime Contractor only. Certification by a subcontractor or teaming partner is not acceptable. If the prime contractor is a Joint Venture (JV) with more than one company making up the JV, one member of the JV is sufficient to count as being certified. No other certification will be acceptable.” Will this evaluation methodology of members within a SBA approved Mentor‐Protégé JV be applied to all aspects of this solicitation to include but not limited to the Scope, Magnitude , Applicability, Complexity, IDIQ, FFP, and Prime Contractor Sections of the Self Scoring Technical Capability Worksheet? For example If the SBA approved Mentor‐Protégé JV is the Prime, is current and past performance of any member considered as Prime current and past performance?
Page 130 See response to questions 3003 and 3013.
3015 Recency On pages 141 ‐ 144 the solicitation states that “work that was performed anytime from 1 Jan 12 through 30 Sep 16” can be used to validate items claimed on Attachment 5. Will a Contract or Task Order with a period of performance beginning before 1 January 2012 but ending after 1 January 2012 or beginning prior to, but ending after, 30 September 2016 meet the requirement or does the entire period of performance need to fall between 1 Jan 12 and 30 Sep 16 in order for it to be valid?
Pages 141‐144 Work for any amount of time during the period 1 Jan 2012 to 30 Sep 2016 would be considered relevant experience for technical capability. The same dates are also used for recency in past performance references.
Performance that ended before 1 Jan 2012 or started after 30 Sep 2016 would not be relevant for technical capability or recent for past performance.
3016 Self Scoring Worksheet
Page 142 3.2 Evaluation Factors Volume II – Technical. Assuming that multiple Offerors earn the maximum point value for items claimed on Attachment 5 in the Relevant Experience, Magnitude, Applicability or Complexity sections of the worksheet; will the government give greater consideration to Offerors listing items that are of a) greater magnitude or
b) are more recent?
Page 142 The maximum point values on the self scoring technical capability worksheet are the maximum possible for each of the areas.
3017 Past Performance Reference
Page 146 IDIQ Contract References. May multiple Task Orders from one IDIQ contract be listed on a single Attachment 6?
Page 146 The past performance questionnaire is designed to provide information on one past performance reference. Multiple task orders on an IDIQ contract will have one past performance questionnaire.
3018 Past Performance Reference
Page 146 IDIQ Contract References. If multiple task orders under an IDIQ contract are listed on a single Attachment 6 will they be considered to be one reference?
Page 146 Yes. See response to question 3017.
er Topic Question/Comment RFP Reference Answer
3019 Past Performance Reference
Page 146 IDIQ Contract References. Would multiple Past Performance questionnaires on the same IDIQ contract be considered one (1) Past Performance Reference?
Page 146 The Offeror would not be able to submit more than one past performance questionnaire on an IDIQ contract as a reference. See response to question 3017.
3020 Self Scoring Worksheet
According to page 130, the second paragraph, the requirement states that Offerors cannot consolidate requirements on multiple task delivery orders.
During industry day this was to be identified in relation to MUTILPLE AWARD CONTRACTS (MACS) or MATOS. Please confirm that the offerors can consolidate task orders for SINGLE AWARD IDIQ CONTRACTS.
Page 130 For claiming items (points) on the self scoring technical capability worksheet, Offerors cannot consolidate requirements on multiple task orders to obtain a higher point value, which includes task orders on both multiple award and single award IDIQ contracts.
3021 Letter of Credit Please confirm that the letter of credit must be from the prime; if offeror is a joint venture, please confirm that the letter of credit must be from the JV itself or the Small Business managing member of the JV.

3022 Self Scoring Worksheet
Please confirm that single‐award ID/IQ or BPA task orders may be consolidated, as stated during the industry day.
Page 130; 7. Volume II Technical;
Technical Capability Supporting Documentation
See response to question 3020.
3023 Past Performance Reference
As it relates to Past Performance References from the “Prime”, please consider the Prime to be the JV itself or the Small Business managing member of the JV, for the minimum one reference.
Page 131; 8. Volume III Past Performance See response to question 3013.
3024 Past Performance Reference
Please confirm that the contract used in the Past Performance Reference submission does not have to be one of the contracts used for claimed items on Attachment 5.
Page 131; 8. Volume III Past Performance There is no requirement to use the same contract from a claimed item (pointed) on the self scoring technical capability worksheet as a past performance reference.
er Topic Question/Comment RFP Reference Answer
3025 Self Scoring Worksheet
Please consider requiring the JV itself or the Small Business managing member of the JV to hold the HCSS certification to receive credit for the claimed item, versus any other member of a JV.
144 & Attachment 5; 3.2 Volume II Technical, HCSS Certification & Attachment 5
There is no restriction on which member of a Joint Venture has the Joint Commission certif ication.
3026 Self Scoring Worksheet
Please consider the “Prime” to be the JV itself or the Small Business managing member of the JV to receive credit for the claimed items of ID/IQ Experience, FFP Experience, and Prime experience.
144 & Attachment 5; 3.2 Volume II Technical & Attachment 5
If the Joint Venture is the Prime, there is no restriction to limiting the point value to the Small Business managing member of a Joint Venture.
3027 Letter of Credit The referenced solicitation states that it is mandatory for us to have a $6M Line of Credit. This number is way too steep for a small business. We are a $28M company and yet only have $1M Credit Line. Banks simply don’t give that large credit lines unless you are a $50M company. We strongly ask you to reduce the mandate on this steep requirement. If not, it would be impossible for the DHA to get many credible offers on this. This requirement is unreasonable and anti‐small business – ironically on a SB set‐aside opportunity.

Pending er Topic Question/Comment RFP Reference Answer
3028 Contractor Team Organization
Are Large Companies allowed as significant subcontractors OR all subs must be Small Businesses?
General Question A large business may team with a small business as long as the prime or joint venture is represented as a small business as outlined in FAR part 19 and the SBA.
3029 Self Scoring Worksheet
Will the Government consider expanding the scope of this line to allow other types of specialized surgeons to satisfy the point criteria for General Surgeons, e.g. Vascular Surgeon or Pediatric Surgeon?
Attachment 5, Line 26: The Offeror has the option of adding one additional labor category on line 35 of the self scoring technical capability worksheet for physicians.
3030 Self Scoring Worksheet
Will the Government allow contractors that provide LPN/LVNs in an Intensive Care Unit to receive points in this category?
Attachment 5, Line 63 LVNs/LPNs are listed on line 65. The Government will amend Attachment 5 for nursing to clearly identify the labor categories as registered nurses. See solicitation Amendment 0001.
3031 Self Scoring Worksheet
If FPDS specifies the contracting agency as the Defense Health Agency, does that render a contract/task order as eligible to receive credit for these points?
Attachment 5, Line 181 Federal Procurement Data Set (FPDS) is not one of the supporting documents the Offeror may submit to support a claimed item on the self scoring technical capability worksheet.
3032 Self Scoring Worksheet
If FPDS specifies the contract type as Firm Fixed Price, does that render a contract/task order as eligible to receive credit for these points?
Specifically referring to contracts where FFP refers to the fixed unit price as is the case on many healthcare task orders.
Attachment 5, Line 234 See response to question 3031.
er Topic Question/Comment RFP Reference Answer
3033 Supporting Documentation
If a contract/task order was issued for 1 FTE at 1880 hours, does that task order receive points in the “>0, but <1” or “≥1, but ≤2” column? Does it matter more the number of FTEs specified in the contract or the number of hours per FTE? Alternatively, if we have an order for 4,000 Hours for a single labor category on one task order, does that order point in the “≥1, but ≤2” or “>2, but ≤6”?
Attachment 5 The supporting documentation must match the item claimed on the self scoring technical capability worksheet (Attachment 5). A task order for 1 FTE would match the item claimed as >= 1, but <=2 if the CPARS shows 1 FTE. The supporting document for a contract showing only 4,000 hours must relate to the number of FTEs the Offeror provided. If the CPARS does not clearly identify the number of FTEs, a Technical Capability Validation Sheet (Attachment 3) will be required for the Offeror to match the claimed item on the self scoring technical capability worksheet.
3034 Supporting Documentation
If the CPARS does not identify the number of FTE or labor categories or other information that specifically identifies information that validates the point criteria, what additional documentation if any is required to justify the number of points that we are claiming?
Page 129 The Offeror is required to provide a signed Technical Capability Validation Sheet (Attachment 3) when a CPARS does not support a claimed item. See response to question 3033.
3035 Recency Regarding the standard that “relevant experience must be for work that was performed from 1 January 2012 through 30 September 2016”, will the Government consider work being performed as credentialing a health care worker on an active task order that was awarded with a start date prior to 30 September 2016?
Attachment 5; Line 17 See response to question 3015.
3036 Deliverable Can the DHA provide a word version of Attachment 3? Attachment 3 See response to question 3001.
3037 Supporting Documentation
Would the DHA please provide an example of how Attachment 3, Section 3 is supposed to look when filled out for multiple rows for a Task Order that has multiple labor categories and locations, and what is supposed to be circled? (i.e.: “Circle row associated with claimed item”)
Attachment 3; Section 3 The multiple rows on Attachment 3 are designed to give the Offeror an opportunity to list multiple labor categories on IDIQ contracts. The Offeror would only need to get the data from the Customer Point of Contact once and then provide the requisite number of copies to support multiple claimed items on the self scoring technical capability worksheet by circling the appropriate row on each copy that corresponds to the specific row on the self scoring technical capability worksheet.
3038 Deliverable Can the DHA provide a word version of Attachment 4? Attachment 4 See response to question 3001.
er Topic Question/Comment RFP Reference Answer
3039 Recency In regard to Volume II‐Technical and Volume III‐Past Performance, would the DHA consider changing the Recent Contracts dates from ending 09/30/2016 to ending 12/31/2016?
Page 130 The recency dates are for performance from 1 January 2012 to 30 September 2016.
3040 PFD File Could the DHA please clarify the section of the Electronic Files Format (Proposal Organization, Page 127) that states “DO NOT submit any documents in PDF format that are copied as “images”. When creating PDF files, the Offeror shall create the file to enable textual search and copy functions.”
1. Any form that needs to be printed and signed, will need to be scanned back in as a PDF image, and in certain cases that PDF may lose the textual search and copy functions.
Page 127, Electronic Files Format (Proposal Organization)
The Government has the appropriate software to search PDF files that were printed, signed, and saved as PDF files.
3041 Proposal Organization
On solicitation page 123 under General Instructions (the first bullet) there is a requirement for offerors to complete the SF 1449 with all amendments signed and “completion of all fillable Provisions and Clauses”. However, throughout the rest of the instructions there is no mention of which Tab those fillable Provisions and Clauses are to be a part of. Can the DHA please clarify where/which Tab the fillable Provisions and Clauses need to be included? Should they be a part of Volume I, Tab 4 Certifications and Representations?
Page 123 A complete SF 1449, to include the amendments and fillable clauses and provisions, are to be provided in Volume I, Tab 2.
3042 Proposal Organization
The instructions for Volume I‐Administration, list the Joint Venture Certification as Tab 4B, but there is no listing for Tab 4A. Is there supposed to be a Tab 4A?
Page 128 See response to question 3009. The Government will amend the RFP to remove the reference to Tab 4B. See solicitation Amendment 0001
3043 Proposal Organization
After reading the solicitation and all proposal instructions, it is our understanding that our submission for Volume II‐Technical Capability will not include any documents titled Tab 2 or Tab 5, only Tabs 2A‐2D and Tabs 5A & 5B. Is this correct?
Pages 128‐130 Tab 2 and Tab 5 are reference tabs. The supporting documents will be listed under Tabs 2A through 2D and Tabs 5A and 5B.
er Topic Question/Comment RFP Reference Answer
3044 Proposal Organization
In regard to Volume II‐Technical Capability, Tabs 2A‐5B, can the DHA please confirm that offerors are required to combine all documentation for each Tab including all CPARS and/or Attachment 3s into one document per Tab? For example, if an offeror is submitting documents for all 14 Physician Market Segment items, the offeror would have 14 different documents (either CPARS or Attachment 3) that are part of Tab 2A. The Offeror would need to combine all 14 documents into 1 document and title that document, Tab 2A.
Page 129 Although the Offeror is authorized to make minor adjustments to the file naming methodology, the Government will amend the solicitation to clearly state that multiple documents provided under one tab shall be labeled sequentially (i.e., Tab2A.1, Tab 2A.2, Tab 2,A.3). See solicitation Amendment 0001.
3045 Proposal Organization
In regard to Volume III‐Past Performance, Tabs 1A‐4C, can the DHA please confirm that offerors are required to combine all documentation for each Tab including Attachments, 6, 7. 8, and 9 into one document per Tab? For example, if an offeror is submitting documents for Physician Reference 1, the offeror would potentially have 4 different documents (Attachments, 6,
7. 8, and 9) that are part of Tab 1A. The Offeror would need to combine all 4 documents into 1 document and title that document, Tab 1A.
Page 131 See response to question 3044.
3046 Proposal Organization
In regard to the PPI tool (and instructions from Attachment 9, Page 6), it is our understanding that offerors are required to provide a copy of the completed PPI tool as part of our proposal submission. The instructions in Attachment 9 state that offerors are to name the file with a Tab # at the end of the file name. If the tool contains past performance reference contracts for multiple Tabs, what is the correct way to name the file?
Attachment 9; Page 6 The Government will add Tabs 1 through 4 to allow the Offerors to place the business relationship printout in the market segment. The business relationship information from the PPI tool must match the Contractor Team Organization in Attachment 4. The Offeror will place the past performance references in the lettered tabs for the appropriate market segment. See solicitation Amendment 0001
3047 Proposal Organization
In regard to the PPI tool (and instructions from Attachment 9), it is our understanding that offerors are to print off a report for the Business Relationships and each Past Performance contract in the tool. Offerors would then attach each past performance report to the corresponding Tab in Volume III‐Past Performance. Can the DHA please clarify where (or under which Tab) offerors are required to provide the Business Relationships report?
Attachment 9; Page 6 The business relationship is an output of the Past Performance Information (PPI) tool and should match the Contractor Team Organization in Attachment 4. See response to question 3046.
er Topic Question/Comment RFP Reference Answer
3048 Past Performance Reference
In regard to Attachment 9, Step 10, please clarify the appropriate responses to this question. Are offerors expected to provide a list of locations under “Place of Work”, or can a statement be provided to state that an entity may provide support at any authorized location?
Attachment 9; Step 10 The information in Step 10 should match the Contractor Team Organization in Attachment 4.
3049 SF1449 In regard to solicitation page 3 (Section SF 1449‐Continuation Sheet) can the DHA please clarify if offerors are required to fill out the CLINs?
1. If the CLINs are required to be filled out and submitted with the offeror’s proposal, can the DHA please specify what Volume and Tab they should be included with?
Page 3 The CLINs are not to be filled out. See response to question 3041. You will price the Sample Market Segment TOPRs (Attachment 2, 2A, 2B, 2C & 2D) IAW solicitation instructions.
3050 Relevancy In regard to solicitation page 131, Table 2, would the DHA please confirm that for Dental Services, the minimum number of labor categories required in order to be consider either Relevant or Somewhat Relevant is one (1)?
Page 131, Table 2 Table 2 lists the minimum number of labor categories for each market segment as one factor to be considered relevant in the relevancy table (Table 4).
3051 Joint Venture In regard to the requirement for a Joint Venture to provide a copy of the Small Business Administration approved Joint Venture certification, will the DHA please remove this language? The SBA All Small Business Mentor Protégé Program does not require any certification of approval of a Joint Venture.
Page 124 See response to question 3009.
3052 Questions Will the DHA accept follow‐up questions based on the answers provided through the first round of Q&A?
General Question The Government does not intend to accept follow‐up questions.
3053 Self Scoring Worksheet
Section 3 of the Technical Capability Validation Sheet has 9 lines for us to provide items claimed on the Self Scoring Technical Capability Worksheet for each contract. What should we do if we need more than 9 lines? Does the Government prefer us to add lines as needed, or should we submit a second Technical Capability Validation Sheet for the additional line items?
Attachment 3 The Offeror shall add rows and number sequentially as needed. The Government provided the MS Word document for ease of use when filling in the required information.
3054 Supporting Documentation
We would like the Government to clarify usage of CPARs submitted with the Self Scoring Technical Capability Worksheet. If we have a CPAR that does not specify the position and/or specific numbers staffed, can the CPAR be used? For example: if the CPAR lists RNs – but not Medical/Surgical Nurses specifically – is the CPAR acceptable?
See response to question 3033.
er Topic Question/Comment RFP Reference Answer
3055 Clauses CLAUSES: 52.203 ‐2, 52,209‐2, 52.209‐5, 52.209‐7, 52.209‐11, and 52.222‐ 22 on Pages 133 through 138 have items which need completion. As the Offeror, Prime Contractor and Signatory, are we required to respond to these clauses and/or complete this Section if we are registered and up to date in SAM?
Pages 133‐138 A complete SF1449 is required. For the fillable provisions and clauses, you may either respond to the provisions and clauses within the SF1449 or attach copies of what you have completed in SAM to the SF1449 submitted in VOL I of your proposal.
3056 Deliverable RFP, page 127. There is a list of documents (second list on the page) that the instructions say "the following documents are limited to the Government specified format".
Question: Attachment 3 and Attachment 4 are furnished as PDF files.
Therefore we cannot adjust the font size to meet the font size requirements of the proposal, and additional rows cannot be added to documents where they need to be added. (e.g. Attachment 4 ‐ the instructions are to add additional rows). Request that contractors be allowed to re‐create the documents as MS word format for completion and signature by our customer.
Page 127 See response to question 3001.
3057 Supporting Documentation
Can a company’s VA 621I Schedule counts as an ID/IQ contract if it’s held by the Prime?
Page 129; ID/IQ Clinical Service Contract Experience
A VA 621I IDIQ contract would count as supporting documentation for the IDIQ Clinical Services Contract experience as long as it is for clinical services.
3058 Past Performance Reference
Can an offeror submit a contract that has (3) different labor categories ( Ex.
Ancillary) , but doesn’t have the space to mark it on the Self‐Scoring Worksheet( Not all Ancillary categories are listed), receive credit as relevant if they are shown in the documentation and all 3 labor categories are on 1 contract?
Page 131; Vol III Past Performance:
Relevant Contract
The Health Care Worker (HCW) labor categories for a past performance reference does not have to match one of the labor categories listed in the self scoring technical capability worksheet, but must match a labor category for the market segment in Exhibit 3.
3059 Self Scoring Worksheet
Why is the only certification allowed for points is TJC; considering most small businesses hold only the small business set aside certifications?
Attachment 3 Technical Capability Sheet The Joint Commission certification for Health Care Staffing Services is closely aligned with the work performed by Offerors on this contract.
er Topic Question/Comment RFP Reference Answer
3060 JC Certification Does an offeror have to be TJC certified to be considered for award? Some large businesses have it, most small businesses do not.
Attachment 3 Technical Capability Sheet See response to question 3003.
3061 Small Business Certification
Considering there are numerous contracts that will be encompassed on this contract vehicle have specific small business set aside categories, will the government consider selecting small businesses that hold certain small business set aside certifications? If so, how will selection be determined?
Page 124, Section 3; Evaluation Process The MQS Acquisition is 100% Small Business Set Aside. After contract awards, the Military Department ordering activities may consider further set asides at the task order level.
3062 Past Performance Reference
If a contract is a coverage contract and has changed contract numbers each year the contract was awarded, should both contract numbers be listed/included?
Attachment 3 Technical Capability Sheet & Attachment 9 Past Performance Information Tool
A past performance reference would include the most appropriate contract number for the reference.
3063 Supporting Documentation
If a contract is a coverage contract and FTEs are not counted, should the contractor note the number of personnel providing services on the contract?
Attachment 3 Technical Capability Sheet See response to question 3033.
3064 Self Scoring Worksheet
In the scoring sheet, should the contractor include a labor category that they currently provide where the labor category is left blank?
Attachment 3 Technical Capability Sheet The self scoring technical capability worksheet allows the Offeror to add one additional labor category in the market segment that matches a labor category in Exhibit 3, HCW Labor Categories.
3065 Past Performance Reference
If a contractor has contracts that did not end until 23 January 2012, can these be included in the past performance tool?
Past Performance Tool See response to question 3015.
3066 Self Scoring Worksheet
Should the offeror fill in "0" or "na" for market segments that the offeror will not be competing for?
Attachment 3 Technical Capability Sheet The instructions require the Offeror to claim items by marking those items with an "x". No other character will work to capture points on the self scoring technical capability worksheet.
3067 Self Scoring Worksheet
If the contractor does not have an ID/IQ contract, will the contractor be considered for award?
Page 3; Evaluation Process An Offeror is not required to claim items in every row of the self scoring technical capability worksheet to be evaluated.
3068 Small Business Goals
How will DHA meet the set aside goals set by the SBA with this contract vehicle?
General Question Small business goals will be determined by the Military Departments (MILDEPs) when soliciting for task orders.
er Topic Question/Comment RFP Reference Answer
3069 Sample TOPR Page 14 of the RFP includes clause 52.216‐22 INDEFINITE QUANTITY. (OCT 1995) (d) Any order issued during the effective period of this contract and not completed within that period shall be completed by the Contractor within the time specified in the order. The contract shall govern the Contractor's and Government's rights and obligations with respect to that order to the same extent as if the order were completed during the contract's effective period; provided, that the Contractor shall not be required to make any deliveries under this contract after 30 September 2027. If a TOPR is released towards the end of the five year ordering period and is a five year TO, will the contractor be allowed to bid updated rates including updated direct labor, overhead, G&A and respective escalation?
Page 14 The basic contract award does not set option year pricing for Task Orders.
All pricing, including option year pricing, will be evaluated and established at the task order level.
3070 PWS Section ‘1.5.6. Recognized Holidays’ Pg. 33; Currently this section does not include a list or number of recognized federal holidays nor do most of the sample TOPRs. Does the Government intend for the standard 10 federal holidays (New Year’s Day, Birthday of Martin Luther King, Jr., Washington’s Birthday, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, and Christmas Day) to be the recognized holidays?
Page 33; Section ‘1.5.6. Recognized Holidays’
Language in paragraph 1.5.6 states that federally recognized holidays will be outlined in the Task Order Performance Work Statement as part of the TOPR. For pricing purposes on the sample TORRs, Offerors shall use 10 federal holidays. See solicitation Amendment 0001.
3071 PWS Section 1.5.7.4.4.1 on page 33 states that the Contractor shall replace permanent HCWs such that vacancies will not exceed 60 consecutive calendar days for privileged and non‐privileged credentialed HCWs and 30 consecutive calendar days non‐credentialed HCWs. The consecutive calendar days starts once the incumbent HCW leaves the position. Can the government please clarify if this time is inclusive of any approvals and credentialing review by the government?
Page 33; Section 1.5.7.4.4.1 The dates are inclusive of approvals and credentialing.
er Topic Question/Comment RFP Reference Answer
3072 PWS Section ‘1.5.8. Place of Performance’ on page 34 states that HCWs shall receive notification two weeks prior to any reassignment. The offeror assumes the reassignment location will be within the 40 mile radius of the original location assigned; is this correct? If this is not correct the offeror assumes the Government will reimburse for mileage outside the 40 mile radius at the prevailing GSA POV rate.
Page 34; Section ‘1.5.8. Place of Performance’
The language in paragraph 1.5.8 specifies the re‐assignment to be within a 40‐mile radius.
3073 PWS Section ‘1.5.18. Emergency Health Care’ Pg. 35; This section currently states the Contractor shall reimburse the Government for emergency medical services provided. Will the HCWs individual health insurance fulfill this requirement?
Page 35; Section ‘1.5.18. Emergency Health Care’
The MTF will capture HCW insurance information for billing from emergencies, when available, or setup a payment agreement. HCWs individual health insurance may fulfill this, in part or in whole. To the extent HCWs individual insurance does not, the contractor shall reimburse the government.
3074 PWS Section ‘1.6.3. Periodic Progress Meetings’ Pg. 36; This section currently does not include frequency of meetings and which personnel are required to attend the periodic progress meetings. Will the Government please identify the estimated number and durations of the meetings and those contractor management personnel (i.e. Project Manager) that will be required to attend?
Page 36; Section ‘1.6.3. Periodic Progress Meetings’
The basic contract has no set interval that the Government and the Contractor may meet beyond the initial conference identified as the minimum guarantee. Task Order Contracting Officers (TOKOs) may require progress meetings pertaining to Task Order requirements.
3075 PWS Section ‘1.6.6. Contractor Travel’ and sub‐section ‘1.6.6.2.’ Pg. 36; Section ‘1.6.6. Contractor Travel’ states reimbursement will not be paid for Contractor travel within a 40 mile radius of the place of performance and sub‐section 1.6.6.2 seems to contradict section 1.6.6. by stating the Contractor will be compensated for mileage, with prior approval, at the FTR/JTR prevailing rate when using a personal vehicle or government provided vehicle for official duties. The offeror assumes that when traveling in official services capacity, excluding commuting mileage, the Contractor will be reimbursed for any and all travel including travel within the 40 mile radius; is this correct?
Page 36; Section ‘1.6.6. Contractor Travel’ and sub‐section ‘1.6.6.2.’
Mileage will not be reimbursed for travel within a 40‐mile radius of the place of performance, unless otherwise stated in the Task Order.

3076 Sample TOPR Section ‘1.6.7. Relocation Costs’ Pg. 37; The solicitation indicates the Government may pay the Contractor to relocate HCWs as specified in the TOPR while the sample TOPRs indicate the Government will not pay the Contractor to relocate the HCWs. In the event the Contractor identifies that relocation cost is needed for the specific location will the Government be willing to discuss and allow reimbursement for relocation if it is determined that qualified personnel require relocation?
Page 37; Section ‘1.6.7. Relocation Costs’ Relocation costs only apply to individual task orders. If relocation costs are determined necessary, the task order ordering activity will include the CLIN in their TOPR. The sample market segment TOPRs provided in Volume IV are for the MQS Acquisition’s evaluation of and will not result in an actual award of a Task Order.
3077 PWS Section ‘4.1 Rubber Stamp’ on page 48 states that the Contractor shall ensure privileged HCWs have a rubber stamp, if required in the TO. In the event the HCW does not have a rubber stamp will the Government reimburse the cost of the rubber stamp as an ODC in the TO?
Page 48; Section ‘4.1 Rubber Stamp’ Part 4 in the RFP is under Contractor furnished items and services.
Therefore, the Government will not reimburse contractor furnished items.
3078 PWS Section ‘5.4.3.2. Malpractice Insurance for Non‐Personal Services’ Pg. 57;
Currently the solicitation does not indicate whether the contractor shall or shall not meet the requirements of FAR Clause 52.237‐7, Indemnification and Medical Liability Insurance. Is it correct to assume that when the final TOPR is released the appropriate box will be checked to indicate if the contractor shall or shall not meet the requirements? And is it the Governments intent that the general liability requirements in the addendum to 52.212‐4(w) Paragraph ‘2. General Liability’ will this general liability insurance meet the requirements identified in 52.237‐7?
Page 57; Section 5.4.3.2. The language in paragraph 5.4.3.2 states that the Task Order will specify the malpractice insurance requirements for non‐personal service arrangement. That same paragraph discusses the requirements regarding the FAR clause 52.237‐7.

3079 PWS Section ‘5.4.4.5. Overage Hours’ Pg. 58; This section indicates that the Contractor is responsible for managing HCW compensation rates applicable at the time and location of specific overage hour use. The offeror assumes there will be reimbursement for overage hours at an overage rate identified in the proposal for the specific TOPR; is this correct?
Page 58; Section ‘5.4.4.5. Overage Hours’ The overage hours are a defined requirement that are beyond the regularly scheduled duty hours. When the Government identifies overage hours on the Task Order, the Contractor would be required to price the overage hours.
3080 PWS Section ‘5.4.4.4. On‐Call Hours’ Pg. 58; This section indicates compensation for on‐call services will be addressed in the TO. The offeror assumes there will be reimbursement for on‐call hours at an on‐call rate identified in the proposal for the specific TOPR; is this correct?
Page 58; Section ‘5.4.4.4. On‐Call Hours’ When the Government identifies on‐call hours on the Task Order, the Contractor would be required to price the on‐call hours.
3081 Sample TOPR Attachment 2 Sample Market TOPRs’– Deliverables Schedule – Can the government please clarify the intent of the Deliverable for Initial Placement of HCWs: Qualifying Documentation (i.e., qualifying package;
credentialing package) and Replacement of HCWs: Qualifying documentation (i.e., qualifying package; credentialing package).
a. Do the days specified in the table apply to the personnel or just to their documentation? If it is their documentation, then would section
1.5.7.4.4.1 on page 33 of the RFP (the Contractor shall replace permanent HCWs such that vacancies will not exceed 60 consecutive calendar days for privileged and non‐privileged credentialed HCWs and 30 consecutive calendar days non‐credentialed HCWs) apply to the personnel unless otherwise specified in the TOPR?
b. If the answer to the above question is the documentation, the deliverable table in the Physician TOPR contradicts with Physician TOPR section 5.2.6.4. Credentialing Package for Privileged HCWs. This states the Contractor shall provide a complete, current, and accurate credentialing package for Pediatrician Physician within 45 days of placement. This is the same for the Ancillary TOPR in regards to the Physical Therapists. Can the government please clarify which one applies?
Attachment 2 The sample market segment TOPRs in Attachment 2 are for pricing evaluation purposes only and do not require submittal of documentation beyond the pricing spreadsheet and, if applicable, justification with rationale for any proposed pricing outside of the Government range. See solicitation Amendment 0001.
er Topic Question/Comment RFP Reference Answer
3082 Sample TOPR Attachment 2 Sample Market TOPRs’– Deliverables Schedule Ancillary/Doctors – Can the government please clarify why the day requirements for qualifying documentation is less for a replacement HCW (15 days vs. 30 days for a new HCW) as it takes the same amount of time for documentation for a new employee vs. filling a vacancy.
Attachment 2 See response to question 3081.
3083 Sample TOPR Attachment 2 Sample Market TOPRs’– Deliverables Schedule– Are the day requirements for qualifying documentation in the sample TOPRs to be expected in the final TOPRs that are issued?
Attachment 2 See response to question 3081.
3084 Sample TOPR Attachment 2 Sample Market TOPRs’ section 5.6.1.4 talks about a Replenish Rate. Can the government please explain what a FTE‐weighted variable is?
Attachment 2 FTE‐weighted variables deal with FTE‐weighted days. FTE‐weighted days are the number of days in a reporting period multiplied by the number of required FTEs in the same reporting period.
3085 Sample TOPR Attachment 2 Price Evaluation Sample Market TOPRs’; ‘Sample Market Segment TOPR: Physicians’ Item number 8 identifies the provision 52.222‐ 46 Evaluation of Compensation for Professional Employees. As this is a sample TOPR for evaluation purposes only, is it the Governments intent for the offeror to provide a compensation plan with the response to this sample TOPR? If this is a requirement, in which volume and section should the offeror provide this information?
Attachment 2 The Government will remove Provision 52.222‐46 from the Sample Market Segment TOPR (Physicians). See solicitation Amendment 0001.
3086 Sample TOPR Attachment 2 Price Evaluation Sample Market TOPRs’; ‘Sample Market Segment TOPR: Ancillary’, Section ‘5.4.4.1. Schedule Management for Personal Services’ and section ‘5.4.4.1 Call‐Back Hours’ have the same section numbering; Will the Government correct the section numbering for ‘Schedule Management for Personal Services’ and ‘Call‐Back Hours’ in the final TOPR?
Attachment 2 The Government will remove call back hours on all the sample TOPRs. See er Topic Question/Comment RFP Reference Answer
3087 Sample TOPR Attachment 2 Price Evaluation Sample Market TOPRs’; ‘Sample Market Segment TOPR: Dental’, Section ‘5.4.4.3. Duty Hours’ states that in no instance a HCW will be required to provide services (scheduled shifts plus leave and holidays) in excess of 80 hours per two‐week period, however Section ‘5.4.4.5. Overage Hours’ states that the HCW shall be compensated in the event that services extend beyond the normal work schedule. These two sections seem to contradict each other. Does the Government intend on allowing HCWs to be compensated for work in excess of the 80 hours per two‐week period in the event that overage hours are necessary? If so is the offeror correct to assume there will be reimbursement for overage hours at an overage rate identified in the proposal for the specific TOPR?
Attachment 2 The Government will remove the overage hours in the dental sample market segment TOPR. The sample TOPR is for evaluation of price only (Step 3 of the evaluation process) and will not result in an actual award of a Task Order. See solicitation Amendment 0001.
3088 Sample TOPR ‘Attachment 2 Price Evaluation Sample Market TOPRs’; ‘Sample Market Segment TOPR: Dental’, Section ‘5.2.9. Security Investigative Requirements’, subsections 5.2.9.2 and 5.2.9.3. reference an attachment, but the attachment is not listed. Will the Government be providing an attachment for these references?
Attachment 2 The Government will not provide security attachments as stated in the sample TOPR. The Government will remove any references to the security attachment being provided from the RFP. See solicitation Amendment 0001.
3089 Contractor Team Organization
Attachment 4 asks for the Location Coverage (List States). Does the government want to know which locations we would like to staff based on the Exhibit 4 or does the government want to know which states we have staffed in before based on our past performance?
Attachment 4 The Contractor Team Organization (Attachment 4) lists the states/locations the Offeror is capable of providing staffing services.
3090 Sample TOPR Attachment 2 Price Evaluation Sample Market TOPRs’; ‘Sample Market Segment TOPR: Dental’, ‘Sample Market Segment TOPR: Nursing’, and ‘Sample Market Segment TOPR: Ancillary’ each reference an applicable Department of Labor Wage Determination, however the referenced Wage Determinations are not the most current revision. The Offeror assumes the Government will incorporate the most current revision of the applicable Wage Determination revision; Is this correct?
Attachment 2 Offeror's will be required to base pricing for the applicable sample market segment TOPRs on the DOL wage determination listed in Attachment 2. See solicitation Amendment 0001.
