Instructions

The offeror must complete this spreadsheet for the purpose of the Government's Price Evaluation. At the top of the worksheet the offeror shall enter the name of their company.

I. FIXED PRICE BREAKDOWN FOR PROFESSIONAL SERVICES

Input the Basic Labor Rates, Fringe amounts, Overhead & G&A amounts, and Profit amounts for each category in the corresponding yellow cells. Take note, the inputs for the Fringe, Overhead &G&A, and Profit columns should be amounts, NOT rates or factors. The following columns automatically calculate and do not require entries: Annual Labor Cost, Labor Price, Fixed Monthly Amount.

II. FIXED PRICE BREAKDOWN FOR ADMINISTRATIVE SUPPORT

Input the Hourly Wage rates, Health and Welfare amounts, Paid Time Off amounts, Tax amounts, WCI amounts, Overhead amounts, ODC amounts, G&A amounts, and Profit amounts into the yellow highlighted cells. The following columns automatically calculate and do not require entries: Total Basic Labor, Total of Labor Cost, Total Cost, Total Price, and the Fixed Monthly Price.

III. ADDITIONAL HOURS FOR PROFESSIONAL SERVICES AND ADMINISTRATIVE SUPPORT

Input the fully burdened labor rates that the offeror intends to bill for each labor category when services are ordered under the Additional Services CLINS. The fully burdened labor rates should be inclusive of all applicable indirect costs (i.e. fringe, overhead and G&A), profit, and any other applicable loadings. It is highly recommended that when developing the fully burdened labor rates, offerors take into consideration the circumstances in which Additional Services can be ordered as described in the SOW.

IV. FIXED INDIRECT RATE APPLICABLE TO TRAVEL

In the yellow shaded cell, input the fixed indirect rate that will be applied to Travel costs associated with the Travel CLIN.

NOTE: - The annual not to exceed Travel amounts ($15,000 per ordering period except for $7,500 in the 6 month extension) are ceiling amounts inclusive of both the travel costs and indirect costs. However, for price evaluation purposes, the Government intends to apply the proposed fixed indirect rates to the annual not to exceed travel ceilings.

ADDITIONAL NOTES:

1. In the event that the fixed monthly prices provided in this worksheet differ from the prices shown on the vendor's SF 1449, the offeror's proposed price for this solicitation shall be considered the extended amounts shown on SF 1449.

2. The data to complete these worksheets is considered other than certified cost or pricing data, and, therefore is not subject to certification as provided under FAR 15.403-1(b)(1) and (3).

3. Offerors may make modifications to the Government provided pricing worksheets, however they must at a minimum provide the price build up information required by original spreadsheets inlcuded in the solicitation. If the Government provided worksheets are altered in any manner, it is the responsibility of the offeror to consider the impact the changes will have on the embedded calculations and formulas and on their price build-up. Accordingly, it is incumbent upon the offeror to ensure that all calculations and formulas result in accurate pricing. A narrative explanation must be provided to explain any additional cost elements added by the offeror and any revisions to the embedded formulas.
