L.11.2 Past Performance Volume and Questionnaires. The Government will conduct a Past Performance assessment based upon the quality and relevancy of the offeror’s past performance, as well as that of its proposed major subcontractors, as it relates to the probability of successful accomplishment of the required effort. The Past Performance evaluation is based on information obtained from the client references and Offeror’s Past Performance Volume. The RFP refers to both a "Past Performance Volume" and "Past Performance Questionnaires." No page limits are provided for Factor 2 Past Performance.
What constitutes the Past Performance Volume (Factor 2 of Volume I)? Is it the governments intention that the PPQ, which are sent to program customer reference POCs, make up the entire Factor 2? or is there other content required in an actual past performance write-up.
Will offerors be permitted to develop a Past Performance Template following page size and font restrictions listed in L.11.1 and past performance requirements listed in L.11.2 and M.3.2? to augment the information in the PPQs

78-79 L.12 RFP indicates our rates are to be blended rates (prime, sub or division), yet the pricing template requires identification in Column B whether the LC applies to prime, sub or division. What information is the Government requesting? Are you looking for a simple confirmation the Offeror understands the rates apply to all Contractor labor whether prime, sub or division? Offerors shall propose a single blended rate for each labor category. The offeror must specify whether the fixed hourly rate for each labor category applies to labor performed by (1) The offeror;
(2) Subcontractors; and/or
(3) Divisions, subsidiaries, or affiliates of the offeror under a common control. If more than one entity will perform the labor category the Offeror shall identify the entities.
