Past Performance Questionnaire(s): Source selection will be heavily weighted in favor of completed, official federal past performance reports (e.g. CPS, CPARS, APARS) with report completion dates no earlier than 3 years prior to the solicitation issue date. Offerors shall not submit any such completed federal past performance reports as they are already retrievable from a Government-wide database.

However, in the event that your firm has not been officially evaluated by a federal agency (for instance, contracts exceeding $150,000 with states, commercial firms, local governments), you are invited to have those customers submit information to the individual in Block 7 of the cover page (SF1449) through the attached questionnaire. Fill out the first 2 blocks and submit to customers to complete, utilizing the attached Rating Guidelines. Have your customer send his/her questionnaire response directly via email to the individual in Block 7 of the cover page (SF1449). Offerors are advised that incomplete or poorly supported information will automatically be considered non-relevant in the evaluation process. Respondents are responsible for ensuring that questionnaires reach the intended destination through confirmation with customers. Respondents shall not seek confirmation of receipt from the Government. Furthermore, respondents shall not ask customers to supply completed questionnaires on projects for which a report already exists in the federal system. SFLC will discard such (duplicative) questionnaires.

Summarize contractor performance in each of the rating areas. Assign each area a rating of
