Question and Answer 1 through 3
Question 1: In section 3. Factor 2, past performance, you state that we need to submit these past performance forms in the submission of our Bid. Is the Vendor responsible for submitting the unevaluated questionnaires within its bid submission or is only the evaluator responsible for submitting the completed questionnaire to the Coast Guard?
Answer 1: IAW section 3, "Offerors shall complete Section II of Attachment 3 and send Attachment 3 to the reference specified in Section II; this shall be completed for each past performance reference. Offerors shall request that each reference complete Section IV of Attachment 3 and return it to the Coast Guard in accordance with the directions in Section I. It is the offeror’s responsibility to ensure the reference contact information in Section II is valid. If the reference does not submit a Past Performance Questionnaire, the reference may not be considered in the evaluation of the offeror’s past performance.
b. The offeror shall complete Section II of the Attachment 3, Past Performance Questionnaire and return the questionnaire to the Government with their proposal."
To clarify, the offeror shall send the unevaluated questionnaire to the requestor in Section I and also provide the same unevaluated questionnaire with their quote to the Government. Further, it is the quoter's responsibility that the evaluated questionnaire is provided to the Government from the requestor prior to closing date/time.
