Attachment C CSPO OT&E Past Performance Questionnaire You have been reported as having recent experience with the provider of Systems Engineering services by the Contractor named below. The U.S. Customs and Border Protection Agency (CBP) is evaluating proposals for an Operational Test and Evaluation Support services acquisition. To help us evaluate the Vendor’s past performance, would you please fill out this short survey and identify yourself at the bottom of the form. Please return it electronically via email to: Jagdish.Narayan@dhs.gov.
The ratings are described as:
| Neutral (N) |
| No relevant performance record Is identifiable upon which to base a meaningful performance rating. A search was unable to identify any relevant past performance information for the offeror or key team members/subcontractors or their key personnel. This is neither a negative or positive assessment. |
| Superior (SU) |
| Based on an offeror’s past performance record, essentially no doubt exists that the offeror will successfully perform the required effort. |
| Satisfactory (S) |
| Based on an offeror’s past performance record, some doubt exists that the offeror will successfully perform the required effort. |
| Unsatisfactory (U) |
| Based on an offeror’s past performance record, extreme doubt exists that the offeror will successfully perform the required effort. |
