About this file
This document contains a quarterly self-assessment report template and monthly status report attachment for a Visual Information Production Center services contract. The quarterly report template requires the contractor to assess performance on quality, schedule, cost control, management, small business utilization, and regulatory compliance factors. For each factor, the contractor must provide its presumptive rating according to the quality assurance surveillance plan, along with details on any identified weaknesses or required corrective actions taken. The contractor must also describe any contractual requirements exceeded and the related benefits provided to the government. The monthly status report attachment appears to be a reporting mechanism to monitor ongoing contract performance. The Missile Defense Agency is soliciting proposals for these Visual Information Production Center services under its Technical, Engineering, Advisory, and Management Support contracts. Offerors must register in the System for Award Management database. Proposals are due 30 days after issuance of the solicitation, which is anticipated to be on June 7, 2019. The contract will be a firm-fixed-price award for a three-year base period and two one-year options.

Attachment J-09 Quarterly Self Assessment
Quarterly Self-Assessment Report (QSR) Format
A current, fact-based self-assessment will include identification of accomplishments, risks encountered and how they were mitigated, and deficiencies in any aspect of contract performance. It will include any corrective actions required, whether or not they were successful, and proactive measures taken to prevent the recurrence of similar situations.
The Contractor may devise its own format for the QSR as long as the report addresses all of the applicable considerations for each of the Contractor Performance Assessment Report (CPAR) factors. Once the initial report is received, the Contracting Officer’s Representative may advise the Contractor of needed improvements to the format of the report.
CPAR Factors: Quality, Schedule, Cost Control, Management, Small Business Utilization and
Regulatory Compliance
Contractor’s Presumptive Rating: List the rating that the Contractor has earned IAW with the Quality
Assurance Surveillance Plan (QASP) and the Guidance for the CPARS available on CPARS.gov.
Identified Weaknesses or Required Corrective Actions: Explain any weaknesses in performance, any corrective actions employed to mitigate the weakness encountered, and the successfulness of the corrective action in resolving the weakness.
Requirement(s) Exceeded: Explain what independent effort was performed that exceeded the
Government’s requirement. Reference the PWS paragraph or other contractual area that was exceeded. Ensure that the subfactors shown in the QASP are addressed.
Benefit to the Government: Explain how the Government benefitted from the exceeded requirement.
Identify the event(s) IAW Enclosure One to the QASP.
