About this file
This document provides instructions for completing and verifying a CORI (Criminal Offender Record Information) form for the Massachusetts Rehabilitation Commission (MRC). The form must be verified in person at an MRC office by a staff member who will review the applicant's government-issued photo ID and sign off on the form. The completed, verified CORI form should then be returned to the Contracts Department of the MRC.
The related state and local contract opportunity is for MRC's Adaptive Housing Services 22 program, which seeks responses to procure accessibility modifications to the principal residences of eligible individuals with severe disabilities. The contract will be awarded by the Commonwealth of Massachusetts and the term is not specified.
