About this file

This addendum document provides floor plans for the Highland Creek Housing & Wellness Campus in San Bernardino, California, a multi-program facility under development by San Bernardino County Purchasing Department. The addendum presents architectural layouts for multiple building components, including Building A which contains single-family residential units with bedrooms, bathrooms, living areas, kitchens, and administrative spaces, along with a classroom facility. The floor plans illustrate the spatial configuration of the 6.82-acre campus located at 2626 Pacific Street, which will comprise 28 permanent supportive housing units, 8 interim housing units, 30 Community Care Expansion PSH units, 32 substance use disorder treatment beds, and 32 recuperative care beds. The RFP (AGENCY26-PURC-6467) requests proposals for property management services and/or recuperative care services, with a mandatory virtual proposal conference scheduled for August 11, 2026, at 10 a.m. PDT, and proposals due September 4, 2026. Contract award is anticipated in October 2026, with a maximum term of three years plus options to extend for up to two additional years.

The County anticipates a financially sustainable operating model without ongoing County subsidy, with proposers expected to demonstrate sustainable reimbursement through CalAIM Community Supports and managed care partnerships. Eligible proposers must have five or more years of relevant experience and submit detailed staffing plans, annual operating budgets, financial statements, insurance documentation, and three professional references. The evaluation criteria include qualifications and experience (15 points), financial capacity (15 points), operational approach (25 points), supportive services coordination (15 points), community relations (10 points), cost proposal (10 points), and references (10 points). California prevailing wage determinations apply to the project. Written questions are due August 14, 2026, at 1 p.m. PDT through the ePro system, with responses and site documentation provided as Amendment No. 3.
