SUMMARY OF CHANGES
SECTION SF 1449 - CONTINUATION SHEET
The following have been modified:
ADDENDUM TO 52.212-1
ADDENDUM TO 52.212-1, INSTRUCTIONS TO OFFERORS:
(h) Points of Contact: The contracting officer (KO) and contract specialist (CS) are the sole points of contact for this procurement. Questions should be addressed to vivianna.m.diaz.civ@mail.mil with cc to
Adelis.M.Rodriguez.civ@mail.mil. Please provide any questions, in writing, no later than August 9, 2018 at
09:00am (EST). The Government reserves the right to not answer questions submitted after this time.
Questions submitted to any parties aside from the KO and CS will not be addressed under any circumstances.
(i) Discrepancies: If an offeror believes that the requirements in these instructions contain an error, omission, or are otherwise unsound, the offeror shall immediately notify the CS/KO in writing with supporting rationale no later than August 9, 2018 at 09:00am (EST).
(j) Electronic Reference Documents: All referenced documents for this solicitation are available on the Federal
Business Opportunities (FedBizOpps) web site at http://www.fedbizopps.gov. Potential offerors are encouraged to subscribe for real-time e-mail notifications when information has been posted to the website for this solicitation. FedBizOpps is the sole repository and authoritative source for all information related to this procurement. The Government is not responsible for the accuracy of content related to this procurement on third-party websites.
(k) Communications: Exchanges of source selection information between Government and offerors will be controlled by the KO and CS.
(l) Notice to Offerors: Funds are not presently available for this effort. No award will be made under this solicitation until funds are available. The Government reserves the right to cancel this solicitation, either before or after the closing date. In the event the Government cancels this solicitation, the Government has no obligation to reimburse an offeror for any costs.
(m) Submission Instructions: Non-conformance with these instructions may result in removal of the proposal from further evaluation. The Government also reserves the right not to consider for award any proposal that does not adhere to the administrative requirements of this RFP.
(1)Offerors are requested to submit a technical, past performance, and a price proposal in response to the attached Performance Work Statement (PWS), see Attachment 1.
(2) Offerors shall complete Blocks 6, 7 and 8 of the DD Form 254, see Attachment 2, with the appropriate information for their companies and any subcontractors (when subcontractors are proposed). The completed DD Form 254 shall be submitted with your proposal.
(3) Information on submission of technical, past performance, and price proposals is provided below.
The proposal shall be clear, concise, and include sufficient detail for effective evaluation and for substantiating the validity of statements asserted in the proposal. The proposal should not simply rephrase or restate the Government's requirements but rather shall provide convincing rationale to address how the offeror intends to meet these requirements. Merely re-stating PWS requirements may mailto:vivianna.m.diaz.civ@mail.mil mailto:Adelis.M.Rodriguez.civ@mail.mil http://www.fedbizopps.gov/ be considered a lack of understanding of the requirements. Offerors shall assume the Government has no prior knowledge of their capabilities and will base its evaluation on the information presented in the offeror's proposal. The offeror's proposal shall include all data and information requested by the RFP and shall be submitted in accordance with these instructions. The proposal should sufficiently address the requirements as stated in the PWS.
(4) IAW DISA Acquisition Regulation Supplement (DARS) 52.209-9000, each offeror shall specifically identify in its proposal whether or not any potential or actual Organizational and
Consultant Conflicts of Interest (OCCI), as described in Federal Acquisition Regulation (FAR) Subpart
9.5, exists for this instant procurement. If the offeror believes that no OCCI exists, the OCCI response shall set forth sufficient details to support such a position. If an offeror believes that an actual or perceived OCCI does exist on the instant procurement, the offeror shall submit an OCCI plan with the proposal, explaining in detail how the OCCI will be mitigated and/or avoided.
(5) Page limitations are identified below and will be treated as maximums. If exceeded, excess pages will not be read or considered in evaluation of the proposal. The page format shall have a 1 inch margin using no smaller than an 11 point Ariel, Times New Roman, or Courier New font for all text to include any text contained in a table. Page limitations shall not be circumvented by including text boxes/pop-ups or internet links to additional information; such inclusions are not acceptable and will not be considered part of a proposal. The following items will not be considered when calculating page limits: table of contents, glossary, resumes, OCCI mitigation plans, past performance references, DD254, FAR/DFARS/DARS Provisions and/or Clause contractor responses, and Section 508
Compliance. In the event any evaluation notices (EN) are issued, page limitations may also be placed on those.
(6) It is the sole responsibility of the Offeror to ensure that the electronic files submitted are virus free and can be opened and read by the Government. Proposal submissions shall not be locked, encrypted, or otherwise contain barriers to opening. The Government is under no obligation to seek clarification regarding electronic proposal submissions if submissions cannot be opened or accessed.
(7) Section 508 Accessibility Standards apply to this requirement. Section 508 requirements are listed in the PWS. Compliance with Section 508 standards will be considered prior to evaluating any of the selection criteria listed below. Any proposal failing to meet the Section 508 Accessibility Standards, as listed in the PWS, shall be eliminated from consideration for award.
(8) The Government intends to evaluate proposals and award a contract without discussions with contractors. However, the Government reserves the right to conduct discussions or seek clarifications if the contracting officer (KO) determines they are necessary. The Government further reserves the right to reduce the number of proposals being competitively considered to the greatest number that will permit efficient competition among the most highly rated proposals. If a contractor’s proposal is removed from further consideration for award, written notice of the removal will be provided to the contractor. In the event issues pertaining to a proposed contracting action cannot be resolved to the contracting officer (KO)'s satisfaction, the Government reserves the right to withdraw and cancel the proposed order. In such event, contractors will be notified in writing.
(9) Portions of the offeror’s proposal may be incorporated into the contract.
(10) The government will not limit cross teaming. The prime contractor can add subcontractors to their team at any time during contract performance however, this requires pre-approval from the
Contracting Officer.
(11) Electronic Offers. The offeror shall submit the proposal in electronic format via email not later than the the time and date specified in the solicitation. The total size of the email (including all attachments) shall not exceed 20MB. If Offeror's submission exceeds 20MB, multiple emails can be used for proposal submission. Each email shall be clearly labeled, with Company name and Volume number. Proposals shall be submitted to the KO/CS identified below:
Vivianna Diaz (Contract Specialist) – vivianna.m.diaz.civ@mail.mil with cc to
Adelis Rodriguez (KO) – adelis.m.rodriguez.civ@mail.mil
(12) Organization. The offeror shall prepare the proposal as set forth in the Proposal Organization
Table below. The titles and contents of the volumes shall be as defined in this table, all of which shall be within the required page limits and within the number of copies as specified. The contents of each proposal volume are described in the information to offerors paragraph as noted in the table below. To the greatest extent possible, each volume shall be written on a stand-alone basis so that its contents may be evaluated with a minimum of cross referencing to other volumes of the proposal. Information required for proposal evaluation which is not found in its designated volume will be assumed to have been omitted from the proposal. Each volume may contain a more detailed table of contents to delineate the subparagraphs within that volume. Additionally, each volume may contain a glossary of all abbreviations and acronyms used, and with an explanation for each. File attachment types with extensions other than .doc, .docx, .xls, .xlsx, .pdf, or .zip may be blocked by email security protocols, and any emails quarantined or unable to be opened as a result of restricted file types will preclude the proposal from being evaluated. Each volume shall be a separate document for a total of seven (7) separate Adobe PDF, word or excel documents, as specified in table 1 below. The price proposal spreadsheet shall be a separate document from the technical proposal and shall be submitted in the MS
Excel spreadsheet provided as an attachment (see Attachment 3). All formulas shall remain present in the spreadsheet for verification.

Any summary material presented in the Executive Summary shall not be considered as meeting the requirements for any portions of other volumes of the proposal.
(ii) Volume II- Technical/Management-Factor A. This volume should be specific and complete, and will be evaluated against the technical/management subfactors, for Factor A as defined in 52.212-2 EVALUATION- COMMERCIAL ITEMS and ADDENDUM TO 52.212-2, EVALUATION- COMMERCIAL ITEMS. This volume shall provide the actual methodology and/or evidence you would use for accomplishing/satisfying the factors/subfactors listed in Factor

(iii) Volume III Past Performance. Each offeror shall submit a past performance volume with its proposal, containing past performance information in accordance with the format contained in the Past Performance References attachment (see Attachment 4) and Past Peformance factor defined in 52.212-2, EVALUATION- COMMERCIAL ITEMS and ADDENDUM TO 52.212-2, EVALUATION- COMMERCIAL ITEMS. The Offeror shall submit present/past performance information with its proposal, in accordance with the format prescribed in Present/Past
Performance References (Attachment 4). If the offeror is a Joint Venture (JV), submission of past performance for the JV partner(s) is also permissible. Offerors will be evaluated on performance under existing and prior contracts for similar products or services. The maximum number of present/past performance references submitted by the Offeror and/or its JV partners may not exceed three (3) total. Recent efforts are those which have been performed during any portion of the last three (3) years, including ongoing efforts which must have been in place at least six (6) months before the proposal due date. (Note: Each individual call order under a Blanket Purchase
Agreement or each task order under an Indefinite Delivery Indefinite Quantity (IDIQ) contract are each considered to be one (1) separate present/past performance effort.)
IMPORTANT: Offerors are advised that they are responsible for providing the past performance questionnaire (PPQ) listed in Attachment 5 to clients and requesting that they complete it and send it back to DISA. The reference information will aid in the Government's evaluation of the offeror's relevant past performance information. The offeror’s references will be required to complete the surveys and return them directly to Vivianna.m.Diaz.civ@mail.mil with a copy to adelis.m.rodriguez.civ@mail.mil, no later than the closing date of this RFP. The offeror is responsible for ensuring that their references have complied with their request. Surveys submitted by the offerors as part of their proposals will not be accepted. The surveys should be sent from the reference person directly to the DISA points of contact listed above. The
Government may follow up with references at its discretion if it does not receive the completed questionnaire or if it has questions about the content of the completed questionnaire.
Recent efforts submitted will be reviewed for relevancy and quality, and then an overall confidence assessment will be given for the past performance factor IAW the Past Performance evaluation table(s) identified in ADDENDUM to 52.212-2, Evaluation of Commercial Items.
A current point of contact for each past performance effort shall be provided by completing the
References section of the Present/Past Performance Information Template (Attachment 4).
References other than those identified by the offeror may be contacted by the Government, and the mailto:Vivianna.m.Diaz.civ@mail.mil mailto:adelis.m.rodriguez.civ@mail.mil information received may be used in the past performance evaluation. Past performance information may be obtained and considered from any other sources available to the Government, to include, but not limited to, the Past Performance Information Retrieval System (PPIRS), Past
Performance Questionnaires (PPQs) (Attachment 5), Federal Awardee Performance and Integrity
Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; interviews with Program Managers, Contracting Officers, and Fee Determining
Officials; and the Defense Contract Management Agency (DCMA).
More recent and relevant performance may have a greater impact on the Performance Confidence
Assessment than a less recent or relevant effort. If a PPIRS entry exists for the proposed past performance effort, the information and ratings contained in the PPIRS entry will take precedence over other sources of information for the Past Performance Evaluation Team. Offerors for whom information on past performance is not available or so sparse that no confidence assessment rating can be reasonably assigned will not be evaluated favorably or unfavorably on past performance and as a result will receive an “Unknown Confidence” rating for the Past Performance Factor.
(iv) Volume IV – Price. This solicitation is being made under anticipated conditions of adequate competition. Should the KO determine that conditions of adequate competition do not exist, and no exceptions apply, the Offeror may be required to provide certification of current cost or pricing data in accordance with FAR 15.406-2. The Government will verify that all solicitation requirements have been priced, figures are correctly calculated, and prices are presented in a clear and useful format. Price submissions should be sufficiently detailed to demonstrate their reasonableness. The price proposal spreadsheet shall be a separate document from the technical proposal and shall be submitted in the MS Excel spreadsheet provided as an attachment (see
Attachment 3). All formulas shall remain present in the spreadsheet for verification. Price proposal shall contain information in accordance with the Price Factor defined in 52.212-2, EVALUATION- COMMERCIAL ITEMS and ADDENDUM TO 52.212-2, EVALUATION-
COMMERCIAL ITEMS. The file size of each document (price proposal and pricing worksheet) shall not exceed 2MB. All information relating to the proposed price, shall be included in the section of the proposal designated as the Price Volume. The eventual awardee will be authorized to invoice monthly for labor and all approved travel costs incurred and approved by the COR.
Under no circumstances shall the information and documentation from the Price Volume be included elsewhere in the proposal other than Volume IV and Volume VII. The Microsoft Excel spreadsheet shall include all formulas and calculations and NOT be hard numbered. Failure to comply with this section may result in the Offeror’s cost submission being determined as non responsive.
The labor rates inputted into the Pricing Worksheet shall include fully-loaded hourly rates for each labor category that may be required throughout the life of this effort. All labor related costs are part of the fully burdened labor rates.
Again, The price quotation spreadsheet (Attachment 3) shall be a separate document from the technical price proposal document.
The Government may require surge support during the base or any option period and surge modifications will be in-scope to provide increased support for the defined task areas of the PWS.
Surge support is OPTIONAL and is not a guarantee. If the Government determines that an increased quantity of support is required for the task areas defined in the PWS, the Government reserves the right to exercise the surge option unilaterally. The Government is providing a percentage value to be used by all offerors when proposing surge: surge support will not exceed
15% of the offeror’s total proposed cost/price for the base and all option periods, excluding any 6-month extension of services pursuant to FAR 52.217-8. Offerors are instructed to propose this
EXACT percentage for Surge CLIN 9999 (see Attachment 3). In the event the Government does elect to exercise the surge option, surge support will be realigned under new or existing
CLINs for the relevant task areas identified in the PWS, and an equal amount will be deducted from optional surge CLIN 9999. Surge support will be provided at the same labor rates proposed and found fair and reasonable at time of contract award for the applicable period of performance.
Due to the likely inability of any offeror to accurately estimate travel associated with this requirement, the Government is providing plug numbers to be used by all offerors when proposing
ODCs. Offerors are instructed to propose plug numbers for ODCs in the following EXACT amounts (see below), however actual travel shall be authorized by the COR in writing prior to performance and will be paid IAW FAR Part 31.205-46.
Travel ODCs
Base Year: $100,000
Opt Year 1: $100,000
Opt Year 2: $100,000
Opt Year 3: $100,000
Opt Year 4: $100,000
52.217-8: $50,000
These EXACT amounts shall include all audited markups and burdens and shall not be rounded or adjusted based upon independent decisions regarding actual anticipated ODCs. A proposal that fails to bid the plug numbers EXACTLY as provided may be excluded from further consideration.
The government considers a full time staff year to be 1,880 hours. Offerors are REQUIRED to use this as a baseline for a full staff year.
(v) Volume V - Contract Documentation. The purpose of this volume is to provide information to the Government for preparing the contract document and supporting file.
1. Offerors are required to complete the applicable provisions that apply to this solicitation. Some of the provisions require a response from the offeror and will need to be submitted along with your proposal.
2. Offerors shall complete Blocks 6, 7, and 8 of the DD Form 254 with the appropriate information for their companies and any subcontractors (when subcontractors are proposed)
(Attachment 2). Completed DD Forms 254 shall be submitted with quotations.
3. IAW DISA Acquisition Regulation Supplement (DARS) 52.209-9000, each contractor shall specifically identify in its quotation, whether or not any potential or actual Organizational and
Consultant Conflicts of Interest (OCCI) as described in Federal Acquisition Regulation (FAR)
Subpart 9.5 exists for this procurement. If the contractor believes that no OCCI exists, the OCCI response shall set forth sufficient details to support such a position. If a contractor believes that an actual or perceived OCCI does exist on this procurement, the contractor shall submit an OCCI plan with the proposal, explaining in detail how the OCCI will be mitigated and/or avoided.
4. A statement that your proposal is valid for at least 120 days.
5. Continuation of Essential Contractor Services Plan.
6. The following administrative requirement applies, in response to the RFP: Evidence of final
Top Secret Facility Clearance, per PWS section 12.1. The Government reserves the right not to consider for award any RFP that does not adhere to the administrative requirements of this RFP.
(vi) Volume VI – Key Personnel Resumes
The offerors shall provide Key Personnel resumes to ensure that they possess the knowledge and expertise necessary to perform the requirements of the PWS as required in sections 6.2.2, 6.3.2, 6.4.2, and 6.5.3 of the PWS. Failure to submit a resume for any of the key personnel categories will deem the proposal non-responsive and the vendor will be excluded from further consideration.
Contractor shall ensure that sensitive Personally Identifiable Information (PII) is omitted from
Key Personnel resumes.
(vii) Volume VII – Price Documentation
In accordance with FAR 15.403-1(b) and 15.403-3(a), data other than certified cost or pricing data may be required to support price reasonableness. Information shall be provided in accordance with
FAR 15.403-5. If, after receipt of proposals, the KO determines the required conditions of adequate competition do not exist and that there is insufficient information available to determine price reasonableness and none of the exceptions in FAR 15.403-1 apply, the Offeror may be required to certify its submitted cost or pricing data (see: FAR 15.406-2 -- Certificate of Current
Cost or Pricing Data). Offeror shall provide whatever information is necessary to help the
Government understand why the proposed prices are reasonable in its Price Narrative Document.
The Offeror shall describe their methodology used to establish their price. The Offeror shall provide information describing and detailing the evaluation it performed to determine the reasonableness of prices on hourly labor rates. The price narrative shall provide whatever information is necessary to help the Government understand why the proposed prices are reasonable. Each Offeror's price proposal shall contain sufficient quantitative and narrative documentation necessary to adequately support and explain the prices proposed.
The price documentation proposal shall be organized as follows:
1. Table of Contents
2. Pricing information, labor rates and supporting data, to include estimating methodology.
