NOTE 2: FOR THE PURPOSE OF EVALUATING THE BID/OFFER FOR THE SAMPLE
PROJECT, THE SIZES, QUANTITIES AND LOCATIONS STATED IN THE
SPECIFICATIONS AND/OR SHOWN ON DRAWINGS SHALL BE USED.
NOTE 3: THERE WILL NOT BE A PRE-BID SITE VISIT / CONSTRUCTION WALK-
THROUGH AS SPECIFIED IN THIS SPEC. HOWEVER, THIS IS A REPRESENTATION
OF WHAT MAY BE REQUIRED IN INDIVIDUAL TASK ORDERS ISSUED AGAINST

Statement of work:
Provide labor, material, and supervision to patch and paint designated walls offices and corridors; in addition, carpet same offices and corridors at the following address. All work to be accomplished after normal hours.

II. Products:
A. All material shall be commercially available first line products that meet or exceed the requirements of associated recognized testing facility.
B. Paint shall be Dunn-Edwards paint ECOSHIELD Low Sheen, low odor Egg Shell, white shadow 181 color. Trim shall be low sheen throughout. Surfaces to be painted shall be repaired and new patches to have one coat of sealer prior to painting.
C. Carpet tile to be, Mohawk MT-085 “In Motion”.
D. 4” Roppe "P-194" Burnt Amber cove base.
III. Miscellaneous:
A. Quality assurance:
1. All material shall be commercially available first line products that meet or exceed the requirements of associated recognized testing facility.
2. All materials, equipment, installation and set-up shall be accordance with this scope of work, all current codes, construction regulations and laws applicable to construction, transport and safety requirements.
3. Engage experienced installers who are licensed and certified for their trade.
4. The contractor shall be responsible for providing the Government with a complete and operable system/project.
5. All materials, equipment and supplies to be incorporated into work under the terms of this contract shall have been “NEW” and “UNUSED” at time of award.
6. All material, workmanship and equipment shall be subject to the inspection and approval by the Contracting Officer and/or their representative.

7. All work unsatisfactorily performed shall be promptly corrected and made acceptable for the Government at no additional charge.
8. Contractor is responsible for all dimensions and quantities [however, see Note 2 on Page 1].
9. Contractor shall field verify all existing conditions prior to submitting a bid [however, see Note 2 on Page 1].
B. Workmanship:
1. All work shall be coordinated and scheduled with the Contracting Officer, and the Contracting Officer has the right to reject any unsatisfactory or unsuitable material or workmanship.
2. Any damaged caused by the contractor to the building, any of its contents or tenants shall be corrected by the contractor in a timely manner as approved by the Contracting Officer at no additional cost to the Government.
3. Contractor shall be given a specified lay down area or storage room for supplies if necessary. GSA will not be responsible for supplies left in this area.
4. All work shall be coordinated and scheduled with the Contracting Officer, and the Contracting Officer has the right to reject any unsatisfactory or unsuitable material or workmanship.
5. Clean up of work site shall be accomplished after each shift of work. Use of Government dumpsters will not be allowed.
6. All work must meet or exceed manufacturer’s specifications.
7. All work shall meet all applicable codes.
C. Licenses and permits
1. The Contractor shall be responsible for any/all permits, fees, taxes or inspections required by local authorities.
D. Scheduling
1. All work shall be scheduled for after hours 5PM to 5AM or normal hours on weekends unless otherwise approved by the Contracting Officer.
2. Contractor shall submit to the GSA Project Manager a project schedule which includes all tasks to be performed under this project, duration of each task and tentative date/s each task will be performed.
3. A project schedule shall be submitted to the GSA Project Manager for approval two weeks prior to start of the project and posted in work areas.
4. The schedule is to reflect and coordinate all aspects of the project including sub-contractors, safety meetings, and inspections.
5. Contractor SHALL be responsible for the all supervision and daily progress reports of this project.
6. Project schedule must be approved by the GSA Contracting Officer prior to performing any work under this project.
7. All work shall be scheduled so it will not interfere with normal Courthouse operations.

8. Microsoft Project or other similar format can be used for this schedule.
9. Contractor has _10_ calendar day to submit all materials and product submittals for GSA and Agency approval.
10. Contractor to notify GSA Project Manager on approximate time of completion to schedule punchlist walk-through.
E. Security:
Security shall be furnished by customer and scheduling shall be planned and organized by General contractor. Security for the project will be conducted through a separate contract.
IV. Execution:
A. Paint: (52,311 SF Approx)
1. Prep wall surfaces of specified corridors and offices.
2. Paint all walls, trim and doors of specified corridors and offices.
3. Remove present carpet of specified corridors and offices and discard debris in a legal dump site.
4. Painters must be qualified journeymen to accomplish the work in a professional manner and with minimum interruptions to the Tenants.
5. Furniture will be moved by third party contracted by the general contractor;
Furniture company will be allowed to begin dismantle of furniture and re-installation of previous day’s work by 5PM every afternoon work is scheduled. Coordination between General contractor and furniture contractor will be imperative.
B. Carpet: (1,860 SY Approx)
1. Remove existing carpet, pad and cove base in areas to be impacted by the contract and discard of debris in a legal dump site.
2. Install new Mohawk MT-085 "In Motion" carpet tile as per manufacturer’s specifications in areas indicated by project manager during the estimate walk.
3. Provide and install new 4” Roppe "P-194" Burnt Amber cove base (4,000 LF approx).
4. Carpet installers must be aware of the manufactures specifications and adhere to its recommendations for installation.
5. Any deviation of the manufacturer’s recommendations must be approved by the project manager prior to installations outside of the manufacturer’s specifications.
C. Completion: 60 days after receipt of notice to proceed
D. Liquidated Damages: $75 per each day of delay.
E. Bonds: Performance and Payment Bonds required

V. Health & Safety: OSHA regulations will be adhered to, and periodic inspections for violations will be conducted. Safety meetings shall be held as required by OSHA regulations and a copy of the sign in sheet and meeting notes shall be provided to GSA project manager for file records. All work shall be coordinated and scheduled with the Contracting Officer, and the Contracting Officer has the right to reject any unsatisfactory or unsuitable material or workmanship
VI. Security Clearances:
(a) Prior to start of work the contractor shall submit a background clearance information form for all employees that will require access to government owned space in performance of this contract. Contract employees determined to be unsuitable will not be allowed access to the work site. Security information shall be submitted to the GSA, Tucson Office after contract has been awarded. Contact Tanya West for information or required security forms.
FAR 52.204-9 Personal Identity Verification of Contractor Personnel (Jan 2006) The Contractor shall comply with agency personal identity verification procedures identified in the contract that implement Homeland Security Presidential Directive- 12 (HSPD-12), Office of Management and Budget (OMB) guidance M-05-24, and Federal Information Processing Standards Publication (FIPS PUB) Number 201.
(b) The contractor shall insert this clause in all subcontracts when the subcontractor is required to have physical access to a federally-controlled facility or access to a Federal information system.

VII. Site Visits [See Note 3]
Site investigations and conditions affecting the work will be included in any contract awarded as a result of this solicitation. Accordingly, all offerors or quotes are urged and expected to inspect the site where the work will be performed.
Contractors are encouraged to attend the pre-construction walk-through.
Contractors are encouraged to verify all existing conditions at the job site prior to bidding on project. Failure to do so will not relieve contractor of his/her responsibility to properly estimate the cost of this project.

Contractors are encouraged to invite their sub-contractors, since this will be the only walk-thru. Please inform this office if you are not planning to attend the site visit. WARRANTY
All workmanship and materials shall be warranted for a period of not less than three hundred sixty five (365) days from date of acceptance by the Government. Contractor shall provide all applicable manufacturer’s warranties at time of completion.
ASSUMPTIONS
For the purpose of price evaluation for this sample project, Offerors shall ADD the following government cost estimate to their price proposal:
1. Furniture moving prior to and after project completion [see IV.A.5.] - $40,000.00
2. Site conditions [see III.A.9.] - $6,000.00

Unlisted classifications needed for work not included within the scope of the classifications listed may be added after award only as provided in the labor standards contract clauses (29CFR 5.5 (a) (1) (ii)).

WAGE DETERMINATION APPEALS PROCESS
1.) Has there been an initial decision in the matter? This can be:

* a survey underlying a wage determination
* a Wage and Hour Division letter setting forth a position on a wage determination matter
* a conformance (additional classification and rate) ruling
