SF 30 Supplementary Page Full Maintenance Services – West Texas Solicitation: GS-07P-12-JU-D-0074 Amendment 03

ALL OFFERORS MUST ACKNOWLEDGE RECEIPT OF THIS AMENDMENT BY COMPLETING ITEMS 8 AND 15 OF THE STANDARD FORM (SF) 30 FOR AMENDMENT 03 AND BLOCK 14 OF THE SF 33 WITH YOUR PROPOSAL SUBMISSION.

The purpose of this amendment is to:

(1) Extend the solicitation close date from Friday, January 11, 2013 at 4:00 p.m. CST to Friday, January 18, 2013 at 4:00 p.m. CST.

The following paragraph within Section A.1. of the solicitation is revised from:

Proposals shall be received no later than January 11. 2013 at 4:00 pm CST. Site visits of the facilities are scheduled during the week of November 26-30, 2012. See Section L for additional information regarding the site visits.

To:

Proposals shall be received no later than January 18, 2013 at 4:00 pm CST. Site visits of the facilities are scheduled during the week of November 26-30, 2012. See Section L for additional information regarding the site visits.

(2) Upload a revised Standard Form (SF) 33. The SF 33 for this solicitation is revised as follows:

A.) The Date Issued shown in Block 5 has been changed from 11/16/2012 to 11/19/2012 in order to accurately reflect the date this solicitation was posted to FedBizOpps.gov.

B.) The Table of Contents located within Block 11 is revised as follows:

a. Section J: Page number has changed from 139 to 146

b. Section K: Page number has changed from 178 to 185

c. Section L: Page number has changed from 199 to 206

d. Section M: Page number has changed from 217 to 224

(3) Revise Section H.18.8. within the solicitation as follows:

H.18.8. Method of Ordering and Invoicing. The COR may order work priced up to $2,500 orally. The CO shall issue a Task Order (GSA Form 300) for work costing more than $2,500. For work/repairs above this $2,500 threshold, the contractor cannot begin work without the GSA Form 300. For emergency situations, the procedures outlined in H.18.9 will be used.

L.5.(a) PAST PERFORMANCE

(1) Past Performance References – 6 Page Limitation The offeror must demonstrate that they have satisfactorily performed relevant services to those described in this solicitation. This requirement may be satisfied by the combined performance of required services under one or more contracts, or through separate performance of required services under multiple contracts. A representative sample of at least 3 but no more than 6 references must be provided. References should be no older than three years after completion of contract performance.

For offers from joint ventures, past performance records of the joint venture and both firms may be considered. An offeror may also submit a past performance record for a joint venture in which they were a party; however, they must clearly specify their role and contribution to the joint venture. In accordance with FAR 15.305(a)(2)(iii), the evaluation will take into account past performance information regarding predecessor companies, key personnel who have relevant experience, or subcontractors that will perform major or critical aspects of the requirement when such information is relevant as long as it is properly identified.

Note: The Government is not limited to only those references provided by the offeror. The Government may also consider the Offeror’s performance on contracts not submitted by the Offeror that the Government is aware of or becomes aware of as a result of reference interviews. Information in the Past Performance Information Retrieval System (PPIRS) will also be reviewed and considered.

The offeror must submit at least 3 but no more than 6 past performance references including the following contract and respondent information only.

(2) Past Performance Questionnaires submitted – No Page Limitation

Past Performance Questionnaires submitted will not be counted as part of the 6 page Past Performance References page limitation.

The Offeror is responsible for sending Past Performance Questionnaires to at least 3 of their customers prior to the closing date of this solicitation. At least 3 Past Performance Questionnaires must be chosen from the Past Performance References submitted above.

The “Past Performance Questionnaire” attached to this solicitation must be utilized for this purpose. See the Microsoft Excel Attachment titled, “Past Performance Questionnaire” FIGURE L-2.

Past Performance Questionnaires must be received by the Contract Specialist by the due date for receipt of proposals.

The Offeror shall e-mail the Past Performance Questionnaires to the Past Performance References and instruct the references to e-mail their responses directly to the GSA Contract Specialist, Matthew Phillips, at matthew.phillips@gsa.gov.

An Optional e-mail instruction is provided below to assist in requesting this information:

We are preparing a Proposal for Full Maintenance Services in the west Texas area. The solicitation requires submission of Past Performance Questionnaires. See attached Past Performance Questionnaire.

In accordance with the solicitation instructions, the following Past Performance Questionnaire must be filled out and e-mailed to the Contract Specialist, Matthew Phillips, at matthew.phillips@gsa.gov.

Please ensure the Past Performance Questionnaire is received by the Contract Specialist on or before Wednesday, December 19, 2012 at 4:00 p.m. CST. The e-mail subject line should be marked “Past Performance Questionnaire – Solicitation for Full Maintenance Services for West Texas”
