General Insurance Requirements Summary
This General Insurance Requirements document outlines the mandatory insurance coverages that contractors and vendors must maintain throughout their contract with the City of Meriden, Connecticut for the Pulaski Gymnasium Floor Replacement project at Casimir Pulaski Elementary School. The contractor must secure and maintain general liability insurance with minimum limits of $1,000,000 per occurrence and $2,000,000 general aggregate, auto liability insurance with combined single limits of $1,000,000, umbrella or excess liability coverage of $1,000,000 per occurrence, and workers' compensation insurance at statutory limits with employers' liability of $1,000,000 per accident, $1,000,000 disease per employee, and $1,000,000 disease policy limit. All insurance policies must be obtained from carriers approved in the State of Connecticut with a minimum AM Best's rating of "A-"VIII, with the City of Meriden named as an additional insured on a primary and non-contributory basis for all policies except workers' compensation. The contractor must also include a Waiver of Subrogation in all applicable policies. Original, completed certificates of insurance must be submitted to the City of Meriden prior to contract issuance, with replacement or renewal certificates provided at least 30 days before policy expiration dates. Any cancellation, limit reduction, or coverage alteration requires 30 days' written notice to the City of Meriden.
For claims-made policies, continuous renewal is required for a minimum of two years following the contract completion date of August 7, 2026. If any claims-made policy is replaced or the retroactive date is changed, the expiring policy must be endorsed to extend the reporting period for claims for an additional two years from project completion. These insurance requirements are essential prerequisites for contract award and ongoing performance under the gymnasium floor replacement project, which is scheduled to commence on or after June 29, 2026, and must achieve substantial completion by August 7, 2026, with liquidated damages of $2,200 per calendar day assessed for delays.
