About this file
This document is a Human Trafficking Affidavit required for compliance with Section 787.06(13) of Florida Statutes, applicable to nongovernmental entities executing, renewing, or extending a contract with a city government. The affidavit is specifically associated with the City of Fort Myers' Annual Paving, Drainage and Concrete project, which requests qualified contractors to provide site/civil construction repairs and improvements on an "as needed basis" throughout the city.
The affidavit requires an authorized representative of a nongovernmental entity to attest under penalty of perjury that the company does not use coercion for labor or services, as defined in the Florida Statutes. The document mandates that if coercion is ever used, the entity must immediately notify the governmental entity, and no further contracts can be executed, renewed, or extended. The form includes spaces for company details, an authorized signature, notarization, and requires the signatory to confirm personal knowledge of the statements and their truthfulness.
