Instructions Form D Price Sheet Instructions The Price Sheet must contain complete responses to all items. If a portion of any section is omitted, the Price Sheet will be deemed Non-Responsive at the sole discretion of the Department.

The Department has divided the state into three (3) Regions. Respondent is required to provide rates for each Region for which the Contractor seeks to perform. Respondent may provide a response for one (1) or more Regions of their choice.

Rates contained within are ceiling rates that may be quoted at a lower rate per task assignment.

Tabs are named by region. Respondent must fill in the prices/rates for the region(s) they wish to be considered for award. Any blank spaces or zero (0) added to any of the cells will result in that service being offered at no cost to the State. To be considered for the Award, Vendor must be able to provide all Services.

This worksheet must be included in both the electronic proposal (original Excel format) and the hard copy proposal (the print area is already pre-set for this worksheet).

Professional Services pay item prices (identified in cells 20-1 through 20-36) are not considered for award.

Reimbursable items are not included in this Price Sheet as rates are not required.

1)Enter Vendor Name in cell B1
2)Enter Rates for each Pay Item for each year of the initial contract term and each year of the renewal contract term.
3)Review Data entries for accuracy.

Review the header information, checking for any formula errors and ensure the rates for each pay item price are accurate.

4) DO NOT ADJUST FORM D, OTHER THAN INDICATED ABOVE

If this form is adjusted, you will be deemed non-responsive.
