About this file
This document is an Addendum No. 5 to a Request for Proposals (RFP) for Petroleum Contamination Site Response Action Services issued by the Florida Department of Environmental Protection (FDEP). The addendum revises the RFP 2024001, Attachment 4 - Required Documentation for Invoicing, Attachment 5 - Subcontractor Request Form, and Form D - Price Sheet. The RFP is for state and local contract opportunities in Leon County and Tampa City, Florida. The revised Attachment 5 provides instructions for ATC contractors to submit a Subcontractor Request Form, including identifying the applicable contract numbers, subcontractor information, and an attestation by the ATC Contract Manager. The addendum requires all respondents to use the amended versions provided by the Department when preparing their responses, as the addendum has replaced previous versions in their entirety.
The RFP is for Petroleum Contamination Site Response Action Services, and the addendum indicates that changes have been made to pricing and other requirements. Respondents must comply with the revised forms and documentation requirements outlined in the addendum. The contract term and any renewal options are not specified in the provided information.
