About this file
This is a Reference Questionnaire form utilized by Collier County Board of County Commissioners Procurement Services Division for the Large Machine Mowing Services Invitation to Bid 26-8620. The form is designed to collect performance evaluation data on bidder firms and key personnel from previous clients. Bidders are required to submit three to ten references from similar projects completed within the last five years, and this questionnaire serves as the standardized evaluation instrument for those references. The form requests basic information including the evaluator's name, company, contact details, and project specifics such as project description, completion date, budget, and project duration. Bids must be submitted electronically through the County's bidding platform by April 13, 2026, and pricing must remain firm for 180 calendar days from the bid opening date. The contract term is three years with two one-year renewal options.
The questionnaire evaluates ten criteria on a scale of 1 to 10, measuring contractor performance in cost management, schedule adherence, work quality, consultative capabilities, personnel management, project administration, communication, risk management, contract compliance, and overall customer satisfaction. References that are not completed or where evaluators lack sufficient knowledge are scored as zero. Historical spending for large machine mowing services ranges from approximately $239,286 to $351,391 annually, with FY 2025 showing the highest expenditure at $351,391.07. Award will be made to the lowest responsive and responsible bidder offering best value, with the County intending to identify Primary, Secondary, and Tertiary awardees. Bidders must possess valid Maintenance of Traffic or Temporary Traffic Control Intermediate Level Certification and maintain appropriate licensing and insurance, including Commercial General Liability with minimum limits of $1,000,000 per occurrence and $2,000,000 aggregate, with Collier County named as additional insured.
