Form 5 Reference Questionnaire Summary
This is a reference questionnaire form used by Collier County Board of County Commissioners Procurement Services Division to evaluate the past performance of firms and key personnel bidding on the Large Machine Mowing Services contract (ITB 26-8620). The form requires bidders to submit three to ten references from clients for whom they have completed similar projects within the last five years. Each reference must be completed and returned with the bid submission by April 13, 2026. The questionnaire evaluates contractor performance across ten criteria including cost management, schedule adherence, work quality, consultative advice, personnel management, project administration, communication, risk management, contract compliance, and overall customer satisfaction. References rate each criterion on a scale of 1 to 10, with blank responses scored as zero. The form collects project-specific information including project description, completion date, budget, and duration to provide context for the evaluation.
The reference questionnaire is a mandatory component of the bid evaluation process for this three-year contract with two one-year renewal options, with the County intending to select Primary, Secondary, and Tertiary awardees. The references must be from completed projects and completed by the evaluator's company representatives who directly worked with the bidding firm. The scoring results will be used in the selection determination to identify the lowest responsive and responsible bidder offering the best value to Collier County, with historical annual expenditures ranging from approximately $239,286 to $351,391. All bid submissions must be made electronically through the County's bidding platform at https://procurement.opengov.com/portal/collier-county-fl.
