About this file
Form 4 - Use of Coercion Forced Labor is a compliance document issued by the Florida Division of Emergency Management (FDEM) for the Emergency Fuels RFP-DEM-24-25-018 solicitation. The form requires nongovernmental entities executing, renewing, or extending contracts with governmental entities to certify they do not use coercion for labor or services as defined in section 787.06, Florida Statutes, and to affirm that commodities offered have not been produced by forced labor. The form is divided into two parts: Part A addresses labor coercion, and Part B addresses commodity production, with both sections requiring signatures from authorized representatives who attest to the statements under penalties of perjury.
The document serves as a critical compliance mechanism to ensure vendors maintain ethical labor practices and transparent supply chains when engaging in state contracts. By mandating these certifications, FDEM aims to prevent potential human rights violations and ensure vendors meet stringent legal and ethical standards. The form requires detailed attestations from company officers or senior management, with space for entity name, representative details, signatures, and dates, ultimately functioning as a sworn declaration of the vendor's commitment to fair and legal labor practices.
