About this file

This is a Reference Questionnaire form used by Collier County's Procurement Services Division to evaluate the qualifications and past performance of firms and key personnel bidding on public transit construction projects. The form requires bidders to provide one completed questionnaire for each required reference, which must be submitted with their proposal. References are evaluated by previous clients who provide scores across ten performance criteria on a scale of 1 to 10, with 10 indicating high satisfaction and 1 indicating low satisfaction. The evaluation areas include ability to manage project costs and schedule, quality of work, quality of consultative advice, professionalism and personnel management, project administration, communication and documentation, risk management, compliance with contract documents and regulations, and overall customer satisfaction. References must provide project details including project description, completion date, project budget, and project duration.

The questionnaire requires references to leave items blank if they lack sufficient knowledge in a particular area, which results in a score of zero for that item. The form collects contact information from the evaluator including name, company, email, fax, and telephone number. All ten criteria must be scored and totaled to provide an overall assessment of the contractor's performance. This reference evaluation process is part of Collier County's selection methodology for establishing a pool of qualified contractors to provide general construction and construction-related services for public transit projects, including bus shelters, structures, roadway and walkway improvements, landscaping, utility work, electrical, plumbing, HVAC, roofing, and signage. Projects are anticipated to be funded through Federal Transit Administration funds, FEMA, Florida Department of Transportation (FDOT), and local county dollars.
