About this file
This is a Reference Questionnaire form issued by Collier County, Florida's Procurement Services Division for use in evaluating vendor proposals submitted in response to an Emergency Catering Services solicitation. The form is designed to collect performance feedback from previous clients of proposing firms and their key personnel, with one completed form required for each of three to ten references submitted by vendors. References must be from similar projects completed within the past five years. The form requests evaluator information, project details including description, completion date, budget, and duration, followed by ten scored criteria assessing the vendor's past performance across cost management, schedule adherence, work quality, communication, professionalism, project administration, risk management, contract compliance, and overall customer satisfaction. Each criterion is rated on a scale of 1 to 10, with 10 indicating high satisfaction and willingness to hire again, 1 indicating dissatisfaction and unwillingness to hire again, and blank responses scored as zero. All reference questionnaires must be fully completed and returned with the proposal submittal by the January 16, 2026 deadline at 3:00 PM.
The Reference Questionnaire form establishes a structured methodology for Collier County to objectively assess vendor performance history as part of the overall proposal evaluation process, which allocates 30 of 100 available points to Experience and Capacity. This evaluation approach supports the county's selection process for awarding Primary, Secondary, and Tertiary emergency catering vendors under a three-year initial contract term with potential one to two two-year renewal options. The use of standardized reference feedback enables consistent comparison across competing proposers and ensures that selected vendors demonstrate proven capability in delivering emergency meal support services, as required for FEMA Public Assistance grant-funded operations.
