About this file
The document is a Foreign Country of Concern Attestation Form (PUR 1355) issued by the Florida Department of Management Services for the MyFloridaNet-3 Core Services Invitation to Negotiate (ITN No. DMS-24/25-260). The form is a mandatory certification required for entities submitting bids, proposals, or replies to contracts that would grant access to personal identifying information. Vendors must attest that their organization is not owned by, organized under the laws of, or principally located in a foreign country of concern, and that no foreign government has a controlling interest in the entity.
The attestation form is part of a larger procurement process for a statewide telecommunications network infrastructure contract. Vendors must complete this form under penalties of perjury, providing their printed name, title, signature, and date to confirm the truthfulness of their statement. This requirement appears to be a security measure to ensure that potential contractors do not have significant foreign government connections that could compromise the integrity or security of the state's network infrastructure.
