About this file
This is a Financial System Risk Assessment form from the U.S. Department of Labor Employment and Training Administration (ETA) used to evaluate applicants' financial management capabilities for federal grant awards.
The form requires applicants to provide organizational information including legal name, founding date, principal officers' details, employer identification number, and employee count. Applicants must document total sales and revenues from the most recent 12-month period and disclose any affiliated organizations that provide services or products related to the grant. The assessment evaluates the adequacy of accounting systems across nine key criteria: whether government agencies have reviewed the system; the type of system (state-administered, internally developed, or web-based); separate identification of receipt and expenditure of program funds by contract/grant; recording of expenditures by project component and budget cost categories; maintenance of time distribution records for employee effort; segregation of direct and indirect expenses; approval status of indirect cost rates; budgetary controls to prevent obligations exceeding available funds by total grant and by budget category; and internal control structures to safeguard grant funds and assets. Additional requirements include disclosure of any legal matters or ongoing financial concerns affecting grant management ability, certification of independent CPA financial statement examinations, provision of a Payment Management System account number for fund placement, and space for supplemental information. Any "No" or "Not Sure" responses to items 2-9 in Section C require detailed written explanation on organizational letterhead signed by a principal officer demonstrating organizational expertise in those areas.
