About this file
This is a Solicitation Modification (Addenda #3) issued by the Commonwealth of Kentucky's Finance and Administration Cabinet, Office of Procurement Services, for the procurement of cloth face masks for COVID-19 prevention. The solicitation seeks to contract with multiple vendors to supply cloth masks meeting specific requirements: one-size-fits-all design, double-layered fabric with elastic ties, adequate coverage of nose and mouth areas, and solid neutral colors (black, navy, or brown). The initial quantity specified is 2,000 masks, though the solicitation indicates that state government agencies will place delivery orders as needed. One sample mask must be submitted prior to the bid closing date of May 8, 2020 at 11:43 a.m. to Debbie Mathews at the Office of Procurement Services in Frankfort, Kentucky. The vendor is responsible for all shipping costs related to sample submission and potential return.
Pricing for the masks must include delivery to all state government agencies statewide, with the actual delivery occurring as individual agencies place orders. Vendors are required to submit samples at their own expense, and samples become the property of the Commonwealth unless a return is requested within thirty days of contract award. The solicitation incorporates all terms and conditions outlined in Attachment A and any subsequent addenda, and vendors must affirm their understanding of these requirements by signing the solicitation document. Failure to comply with the solicitation provisions may result in vendor ineligibility from further participation.
