About this file

This is a FEMA Elevation Certificate template document associated with a Hazard Mitigation Grant Program (HMGP) project administered by the City of New Smyrna Beach, Florida, in partnership with the Federal Emergency Management Agency (FEMA). The project involves the elevation of six single-family residential properties located in Special Flood Hazard Areas (SFHA) classified as Zone "AE" within Volusia County. The scope of work includes comprehensive turnkey services encompassing structural assessments, boundary surveys, geotechnical assessments, construction plans development, permitting, utility disconnection and reconnection, and elevation of structures to 10 feet above existing ground level. The six properties are situated at 455 DeSoto Drive, 645 Goodwin Avenue, 647 Goodwin Avenue, 508 South Indian River Road, 529 South Peninsula Avenue, and 640 South Pine Street. A non-mandatory pre-bid meeting is scheduled for December 9, 2025, at 2:00 PM via Microsoft Teams. Written questions must be submitted by December 18, 2025, at 5:00 PM through the City's eProcurement portal. Bids are due electronically by January 8, 2026, at 2:00 PM, with bid opening occurring at 3:00 PM the same day. All work must be completed by October 31, 2026, in compliance with FEMA grant requirements and applicable building and flood mitigation standards.

The project is 75% federally funded through FEMA's Hazard Mitigation Grant Program (HMGP Contract No. 4337-373-R-b), with the remaining 25% funded by the City of New Smyrna Beach and homeowners. Bidders must submit a bid security of 10% of the total bid amount and maintain specific insurance coverage including Workers' Compensation, Commercial General Liability ($1,000,000 per occurrence), Automobile Liability ($1,000,000 combined single limit), and Professional Liability ($1,000,000). Contractors must be registered with the Florida Department of State, licensed in Volusia County, maintain active Sam.Gov registration, and comply with E-Verify requirements. The procurement encourages participation from disadvantaged business enterprises, requiring contractors to take affirmative steps to include minority business enterprises (MBEs), women's business enterprises (WBEs), veteran-owned businesses (VOBs), and labor surplus area firms. Liquidated damages are assessed at $1,694 per calendar day for work not completed by the specified deadline.
