About this file

The document is a Bid Submission Checklist for the FAC104 Landscaping Products, Parks and Recreation Equipment and Related Products, Supplies and Services contract opportunity issued by the Commonwealth of Massachusetts Operational Services Division. The checklist outlines the mandatory documents required for bid submission, including the Bidder Response Form, Price Sheet, Required Forms, Organizational Chart, Certificate of Good Standing, Business Reference Letters, and documentation of playground equipment and poured-in-place safety surfacing installations. Bids must be submitted with the specified file names and types. The contract term is not provided, but the checklist indicates this is an RFR (Request for Response) process.

The checklist does not provide details on pricing terms, set asides, incumbents, funding requirements and sources, or other salient information about the contract opportunity. It focuses solely on outlining the mandatory bid submission requirements.
