SECTION 4: EVALUATION
Please indicate your satisfaction with the contractor’s performance by placing an “X” in the appropriate block using the scale provided to the right of each question. This scale is defined as follows:
CODE PERFORMANCE LEVEL
E EXCEPTIONAL - The contractor’s performance meets contractual requirements and exceeds many (requirements) to the government’s benefit. The contractual performance was accomplished with few minor problems for which corrective actions taken by the contractor were highly effective.
V VERY GOOD - The contractor’s performance meets contractual requirements and exceeds some (requirements) to the government’s benefit. The contractual performance was accomplished with some minor problems for which corrective actions taken by the contractor were effective.
S SATISFACTORY – The contractor’s performance meets contractual requirements. The contractual performance contained some minor problems for which corrective actions taken by the contractor appeared or were satisfactory.
M MARGINAL – Performance does not meet some contractual requirements. The contractual performance reflects a serious problem for which the contractor has not yet identified corrective actions or the contractor’s proposed actions appear only marginally effective or were not fully implemented.
U UNSATISFACTORY – Performance does not meet most contractual requirements and recovery is not likely in a timely manner. The contractual performance contains serious problem(s) for which the contractor’s corrective actions appear or were ineffective.

Organization Resource Management O1 Ability to provide (and retain) qualified trained personnel that are knowledgeable and able to meet program milestones and requirements, provide problem resolution, communicate with customers, and report program status.
O2 Ability to document, maintain inventory controls, and accountability of material, furnishings, and Government-Furnished Property (GFP).
O3 Ability to fill vacant positions in a timely manner to meet PWS requirements.
Program Management P1 Knowledge and ability to manage Nuclear Weapons Related Material (NWRM) program and processes to support critical daily operations.
P2 Ability to manage and Individual Protective Equipment Element (IPE) requirements critical to supporting base operations and deployment taskings.
P3 Ability to manage and maintain accountability of base level weapons.
P4 Ability to manage the number of personnel necessary to handle unforeseen program requirements (i.e., workload surges, variables in workload requirements, and contingency operations , etc.).
P5 Ability to manage Air Force Equipment Accountability Element (EAE) Requirements.
Quality Control Q1 Ability to investigate and resolve customer complaints in order to improve overall contract performance.
Q2 Ability to execute contractor quality control plan, ensuring effectiveness of all processes identified in the PWS and related materiel management guidance.
Transition Plan T1 Ability to deliver seamless, uninterrupted services and resources during contract phase in.
T2 Ability to provide all deliverables identified in the PWS during the transition period without delays.
T3 Ability to anticipate and mitigate the inherent risks associated with the transition process as a whole to ensure minimal disruption, cost growth, and transition schedule deadline extensions during the transition period.
Price Performance C1 Ability to provide above satisfactory services or deliverables within the constraints of a Firm Fixed Price contract.
C2 Ability to complete tasks and/or projects within proposed prices.
C3 Ability to submit timely proposals for additional or unscheduled requirements and perform within proposed prices.
C4 Ability to alert the government of unforeseen costs before they occur.
